Established in 2018, LIFEPlan CCO NY collaborates with service providers through a Health Home model to connect members with a comprehensive spectrum of services tailored to their unique needs.

Services Offered

LIFEPlan CCO NY provides a range of services designed to support individuals with IDD:

• Care Management Services: Care Managers assist members in finding, coordinating, and accessing services such as healthcare, personal services, social support, chronic disease management, and preventive care.
• Enrollment Assistance: Specialists guide families through the enrollment process for disability services, including community habilitation, respite, employment, housing accommodations, and healthcare.
• Health Home Care Management: Combining developmental disability services with health and wellness services, this program offers more options, greater flexibility, and better outcomes for members.
