This opportunity is for the position of a Full-time Clinician (Nurse Practitioner or Physician Assistant) working with Signify Health, a part of CVS Health. The role involves providing In-Home Health Evaluations to health plan members primarily residing in Maine. This traveling position requires clinicians to visit patients in the comfort of their homes to conduct comprehensive health assessments. The clinician will typically travel throughout the assigned geographic coverage area, which may involve overnight stays, car rentals, or flights depending on business needs. Holding multiple state licenses is a requirement to cover various areas as dictated by the organization's demands.
Clinicians in this role will play a pivotal part in building trusted relationships with patients, offering flexible, comprehensive, and personalized health evaluations directly at patients' doorsteps. Each visit usually lasts about 45 minutes, providing ample time for one-on-one connection and thorough health consultations. The evaluation process includes reviewing the patient's medication and medical history, conducting a physical exam, and administering additional diagnostic tests if ordered by the patient’s health plan, such as diabetic eye exams and spirometry. This job does not require the clinician to prescribe medicines, order lab tests, or change existing treatment regimens, allowing the clinician to focus purely on assessment and patient engagement.
