The Home Health Registered Nurse role at CenterWell involves delivering skilled nursing care to patients within their residence. Reporting directly to the Clinical Manager, nurses provide personalized, one-on-one care that includes medication administration, wound care, patient assessments, and education about disease management and treatment options. Nurses collaborate closely with physicians and other healthcare professionals to develop and implement individualized care plans designed to support patient recovery and maintain health. Monitoring patient condition changes and maintaining accurate, detailed records are essential components of this role. Additionally, Home Health Registered Nurses oversee the activities of Home Health Aides, ensuring proper completion of documentation and quality of care.

This full-time position offers a 40-hour workweek with a pay range estimated between $77,200 and $106,200 annually. Compensation is structured as pay per visit or unit rate, ranging from $49.00 to $69.00. CenterWell Home Health places a high value on ensuring safety, requiring Registered Nurses to participate in tuberculosis screening and to maintain a valid driver’s license, auto insurance, and reliable transportation to accommodate visits to patients' homes.
