As a Home Health Licensed Practical Nurse (LPN) at CenterWell Home Health, you will play an essential role in delivering hands-on nursing care in patients’ homes, working under the direction of a registered nurse and collaborating closely with a multidisciplinary team. The position involves managing individualized care plans that support patient recovery and independence, providing education to patients and their families about disease management and treatment options, and maintaining accurate and timely documentation of patient visits and clinical outcomes. You will act as a vital link between patients, families, physicians, and clinical managers, facilitating communication and care coordination to address any changes in patient conditions or needs promptly.
This full-time position offers a competitive salary range of $53,000 to $73,000 annually, reflecting a commitment to fair compensation based on experience, skills, and geographic location, alongside a per-visit pay range of $33.00 to $47.00. Additionally, there is an attractive $5,000 sign-on bonus to welcome new team members to CenterWell. Job Requirements
- Current nursing license in the practicing state
- Valid driver's license auto insurance and reliable transportation
- Current CPR certification
- Two years experience as an LPN/LVN in a clinical setting
Job Qualifications

Job Duties
- Provide skilled nursing care to patients working onsite in their homes under the direction of an RN
- Implement individualized care plans in collaboration with the patient family and healthcare team
- Educate patients and their families on disease management treatment options and self-care techniques
- Maintain accurate records of patient care including documentation of patient visit interventions and outcomes
- Maintain contact with patients physicians clinical managers and healthcare team regarding patient or family needs and status changes
- Participate in care coordination activities and discharge planning
