The role of the Community Liaison at Saint Michael's Hospice is integral to our outreach and patient engagement strategies. This position is designed for a compassionate, driven individual who excels in relationship-building and communication. As a Community Liaison, you will serve as the essential link between our hospice services and the wider community including families, patients, and various care providers. Your primary focus will be to support patients and families during the critical pre-admission phase by providing clear, empathetic explanations of hospice eligibility, available services, and support options. You will also be responsible for cultivating and maintaining strategic partnerships with key referral sources such as hospitals, skilled nursing facilities, assisted living communities, and physician offices. Furthermore, your role involves promoting hospice services within the community by attending networking events and organizing facility-based activities aimed at raising awareness and encouraging referrals. Coordination with internal teams including admissions, clinical, and administrative staff ensures a smooth and compassionate onboarding process for new patients. This high-impact role offers an opportunity to make a meaningful difference in the lives of patients facing end-of-life challenges by enhancing access to hospice care through education, advocacy, and collaboration.

Job Requirements

  • High school diploma or equivalent
  • Prior experience in healthcare or social services preferred
  • Strong communication and interpersonal skills
  • Ability to manage multiple tasks efficiently
  • Knowledge of community resources and healthcare networks
  • Proficiency in Microsoft Office and CRM software
  • Ability to travel locally for community events
  • Commitment to patient-centered care and confidentiality

Job Duties

  • Serve as the primary point of contact for patients and families during the pre-admission phase
  • Clearly explain hospice eligibility, services, and support options with empathy and professionalism
  • Build and maintain strong relationships with referral sources, including hospitals, skilled nursing facilities, assisted living communities, and physician offices
  • Attend community and networking events to promote visibility and grow referral partnerships
  • Coordinate with the admissions, clinical, and administrative teams to ensure seamless onboarding of new patients
  • Organize and participate in facility-based events to increase awareness and engagement
