This role involves frequent outreach and education efforts aimed at improving health outcomes and fostering stronger relationships between patients and healthcare providers. Working under the supervision of a designated professional, the CHW supports members by offering assistance in navigating the healthcare system, sharing valuable medical information, and connecting individuals to community resources. This position requires flexibility and adaptability, as CHWs often work in varying and sometimes unpredictable environments, such as patient homes and diverse office settings.\n\nThis full-time role, typically scheduled Monday through Friday from 8:00 a.m. to 5:00 p.m. with some flexibility, demands up to 70 percent travel within a 90-mile radius of Cook County. CHWs need reliable transportation and a willingness to engage with a wide range of constituents across diverse cultural, ethnic, and social backgrounds. The ability to effectively communicate, manage multiple priorities, and exercise sound judgment is critical for success in this role. CVS Health offers a competitive hourly pay range from $18.50 to $38.82, which may include bonuses or incentives.\n\nBesides delivering direct support to socially and medically complex patients, CHWs influence healthcare interventions by advocating for adjustments based on members' cultural or religious preferences. Job Requirements

  • must be located in Cook County or surrounding area
  • must be willing to travel within a 90-mile radius from Cook County at least 70 percent of the time

Job Qualifications

  • high school diploma or G.E.D
  • 1-3 years relevant experience such as community outreach or home visits
  • experience serving Medicaid populations in urban or rural settings
  • familiarity with local formal and informal resource networks
  • experience working in community or outpatient settings preferred
  • community health worker certification or equivalent training and experience preferred
  • skills in mental health first aid, trauma-informed care, HIPAA, motivational interviewing preferred
  • experience with computer applications including Excel and electronic health records
  • meet local and state requirements such as first aid, CPR, and TB screening
