At New Jersey Mentor, the focus is on empowering young individuals through a mission-driven approach that values making a tangible difference in the lives of individuals and families. The Clinical Coordinator role is a pivotal position within this framework, requiring clinical skills to manage and provide case management and counseling services for clients with developmental disabilities or mental health issues. The role operates out of the Somerset office, primarily serving community-based areas including Essex, Union, Somerset, and Monmouth counties. This role offers a salary range of $55,000 to $60,000 and is designed for individuals passionate about contributing to a supportive and collaborative team environment.
As a Clinical Coordinator, the successful candidate will work directly with assigned caseloads, conducting assessments, developing, coordinating, and evaluating service plans in collaboration with treatment teams, funding sources, and key stakeholders. The role also requires maintaining accurate and comprehensive records in line with regulatory and contractual standards. Additionally, the coordinator will provide ongoing supervision and consultation to mentors, including participating in pre-service introductions, continuing education, and group meetings with mentors and family homes. This position also involves crisis intervention support, emergency coverage, and support during legal, licensing, and certification activities as required.
The Clinical Coordinator plays a crucial role in enhancing and building an active network of mentors, fostering relationships and support systems that benefit the individuals served. This role demands a self-motivated, detail-oriented professional capable of multitasking and driving results through effective team collaboration. A valid driver’s license and access to a registered vehicle with insurance proof are necessary, especially when transportation to service recipients is involved.

- Master’s degree in social work, counseling, psychology or related field required
- two years of related experience working with individuals with developmental disabilities or mental health issues
- self-motivated and detail-oriented with ability to multi-task

- valid driver’s license in good standing with access to a registered vehicle with proof of insurance if providing transportation to individuals receiving services

Job Duties
- Provide case management and counseling services to an assigned individual caseload
- assess referred individuals and lead development, coordination, and evaluation of service plans in conjunction with treatment team, funding source, and key stakeholders
- maintain current and complete records in accordance with corporate, contractual, and regulatory standards
- provide ongoing monitoring and consultation to mentors regarding implementation of service plans, including pre-service introduction and continuing education
- make mentor and family home visits and conduct mentor group meetings as required
- assist with emergency coverage and crisis intervention support, including participation in rotational on-call system
- assist with legal, licensing, regulatory and certification activities as needed
- participate in activities designed to enhance and build an active network of mentors
