The Associate Center Clinical Director role at ChenMed is a critical leadership position responsible for overseeing and training primary care providers (PCPs) within an assigned center. This role plays a pivotal part in maximizing the delivery of the company’s core care model by enhancing clinical quality, efficiency, and overall patient and clinician satisfaction. The Associate Center Clinical Director balances administrative leadership with hands-on clinical responsibilities, dedicating part of their time to direct patient care and clinical duties depending on the size of the center and number of direct reports. This blend of management and clinical practice ensures that the director is both a leader and an active clinician contributing to the health of the senior patient population.

The position involves comprehensive accountability for the assigned center’s clinical outcomes, requiring the director to mentor and manage PCPs to achieve outstanding clinical results. Beyond individual patient care, the Associate Center Clinical Director leads care teams consisting of care promoters, care facilitators, and care coordinators to provide coordinated, patient-focused care. This role also demands collaboration with hospitalists, transitional care teams, and other healthcare resources to ensure continuity and high standards of care across different healthcare settings such as hospitals, skilled nursing facilities, and home care.

Leadership responsibilities extend to conducting weekly meetings such as Super Huddle and Transforming Care Meeting, supporting center management in financial and operational goals, and taking an active role in recruitment and retention of high-quality clinical staff. The director is expected to foster a culture of engagement, teamwork, and clinical-operations synergy within the center, working closely with Center Operations Directors and Market General Managers. The role requires a strong commitment to excellence in patient care, clinical outcomes, and team development.

Job Requirements

  • MD or DO degree in Internal Medicine, Family Medicine, Geriatrics, or similar specialty
  • current and active MD licensure in state of employment
  • minimum 1-year clinical experience preferred
  • board eligibility required, board certification preferred
  • current DEA number for controlled substances
  • BLS certification within first 90 days
  • proficiency in Microsoft Office
  • ability and willingness to travel locally and occasionally regionally and nationally
  • fluency in English verbal and written
  • independent judgment and decision-making skills

Job Qualifications

  • MD or DO in Internal Medicine, Family Medicine, Geriatrics, or similar specialty required
  • Current, active MD licensure in State of employment required
  • Minimum 1-year clinical experience in geriatric, adult or family practice preferred
  • Board certification in Internal Medicine, Family Medicine, Geriatrics or similar specialty preferred, Board Eligibility required
  • Maintains board certification through MOC, CME or retaking board exams
  • Current DEA number for schedule II-V controlled substances
  • Basic Life Support (BLS) certification from American Heart Association or American Red Cross required within first 90 days of employment

Job Duties

  • Independently provides care for patients with acute and chronic illnesses encountered in older adult patients
  • Takes full accountability for patient care and outcomes and appropriately seeks consultation from specialists when needed
  • Engages with the hospitalist whenever one of their patients is in the hospital
  • Responsible for assessment, diagnosis, treatment, management, education, health promotion and care coordination and documentation for patients with acute and complex chronic health needs
  • Leads their care team consisting of care promoter, care facilitator, and care coordinator for patients able to come to the office
  • For patients unable to come to the office, engages with the transitional care team and other resources
  • Leads Super Huddle and Transforming Care Meeting weekly, supports Center Manager/Market Clinical Leaders as needed
  • Plays an active role in center management and covers for other providers as needed
  • Manages, mentors, and coaches PCPs in assigned center
  • Partners with Center Operations Director/Market General Manager to improve center financial performance and membership
  • Performs other duties as assigned
