The caregiver role at Exceptional Care is crucial to fulfilling this mission. We are currently seeking experienced, compassionate caregivers with a background in dementia and complex case management to join our team. The position offers the opportunity to work flexible shifts including 4, 8, 12, or 24-hour shifts, with a strong preference for candidates who can drive clients to appointments. As a caregiver specializing in dementia and complex care, you will provide critical hands-on assistance to individuals facing cognitive and health challenges such as Alzheimer's disease and other related conditions. Your daily responsibilities will include following personalized care plans, providing emotional support and reassurance, ensuring medication compliance, and responding attentively to the unique behaviors and needs associated with dementia and complex health cases. You will support patients with activities of daily living including bathing, grooming, dressing, and mobility assistance while maintaining a safe and comfortable environment. Additionally, you will engage clients in mental stimulation activities such as puzzles and reminiscing to enhance emotional well-being and reduce feelings of isolation.

In this role, you will also help clients with meal preparation tailored to their dietary needs, promote proper nutrition and hydration, and manage household tasks such as cleaning and laundry to maintain an organized living space. Documentation of completed tasks and any changes in the client’s physical, cognitive, or emotional state is vital, and you will promptly report these updates to the care manager to ensure continuity of care. The agency also provides ongoing training, coaching, and team support to help you succeed and grow in your caregiving career.

Job Requirements

  • High school diploma or equivalent
  • must drive have a California driver license and own vehicle
  • must be willing to drive clients and run errands
  • must be registered on the home care aide registry
  • must complete a criminal background check
  • must be free from health problems that may be injurious to client self and co-workers and present evidence to substantiate this
  • ability to provide hands-on care for extended periods up to 8 hours
  • physical strength and endurance

Job Qualifications

  • Minimum of one to two years of formal caregiving experience specifically with older adults
  • CNA or HHA preferred
  • experience in Alzheimer’s and dementia care
  • ability to handle complex situations
  • compassion patience and empathy
  • strong communication and interpersonal skills with good command of English
  • problem recognition and communication skills
  • knowledge of safety procedures to prevent accidents
  • Hoyer lift transfer skills a plus
  • hospice experience a plus
  • catheter bag management a plus

Job Duties

  • Follow personalized care plans for individuals with dementia
  • provide emotional support and reassurance to patients
  • monitor medication compliance ensuring adherence to prescribed schedules
  • understand and respond to unique needs and behaviors associated with dementia
  • work with patients who have complex health conditions
  • collaborate with care managers to deliver effective care
  • observe clients and report any changes in their health conditions

  • engage patients in mentally stimulating activities such as puzzles and reminiscing
  • provide companionship to reduce feelings of isolation
  • support social connections with friends and family
  • prepare nutritious meals tailored to dietary requirements
  • encourage proper nutrition and hydration
  • manage household tasks including cleaning and laundry
  • maintain a tidy and organized living space
  • document completed tasks and report changes in client’s physical condition mental capability or behavior
  • promptly report any change in client condition to the care manager
  • attend in-service as required by regulation
