The role of Care Manager, also known as the Designated Care Manager, is a vital position within the Sunrise of Carmichael community. This role demands providing the highest level of quality care and services to a designated group of residents, attending to their physical, emotional, and social needs in accordance with their Individualized Service Plans (ISP). The Care Manager ensures that every resident experiences a safe, comfortable, and home-like environment, supporting their independence, dignity, and wellbeing. Responsibilities include hands-on care, monitoring changes in residents' health and behavior, assisting with activities of daily living, and engaging residents in enriching life skills and reminiscence activities. The Care Manager also plays a critical role in risk management and safety, adhering to occupational health guidelines and promoting a clean and hazard-free community.

Job Requirements
- High school diploma or GED
- CPR certificate and first aid as required by state or provincial regulations
- Minimum age of 18 years
- Preferred experience with seniors
- Ability to make decisions and act in resident's best interest
- Commitment to serve seniors
- Ability to comply with safety protocols
- Willingness to complete required training and certifications
- Pass drug test, tuberculosis test, physical evaluation, and background check
- Covid-19 and influenza vaccination if mandated
- Driver’s license and adherence to driver job description if driving a community vehicle
Job Qualifications
- High school diploma or GED
- CPR certificate and first aid training as required by regulations
- At least 18 years of age
- Previous experience working with seniors preferred
- Strong desire to serve and care for seniors
- Ability to make decisions in the resident’s best interest
- Understanding of safety and regulatory compliance
- Effective communication and interpersonal skills

- Good judgment, problem-solving, and decision-making skills
Job Duties
- Review daily log and document pertinent resident information
- Participate in development and updates of Individualized Service Plans (ISP)
- Monitor and care for designated residents during shifts
- Observe, report, and document changes in resident conditions
- Attend daily crossover meetings and communicate resident care needs
- Respond promptly to calls and emergencies
- Greet and assist guests, families, and team members
- Assist with continence management and maintain sanitation
- Encourage and assist residents with activities of daily living and enrichment activities
- Maintain safety regulations and practice standard care precautions
- Partner with team on risk management and occupational safety compliance
- Report accidents and unsafe conditions immediately
- Maintain cleanliness of resident rooms and communal areas
- Provide dining service and ensure pleasant dining experiences
- Implement reminiscence neighborhood care activities and promote resident engagement
- Participate in team meetings and training
- Comply with infection control and safety protocols
- Perform other duties as assigned
