Job Requirements
- Must hold a bachelor's degree in relevant field or equivalent experience
- Certified Medical Assistant (CMA) with Associate's degree acceptable
- Significant mental health-related experience without degree acceptable
- Minimum of two years mental health field experience preferred
- Valid Washington state driver's license
- Current First Aid and CPR certification
- Hold appropriate licensure related to qualifications
- Completion of DSHS background clearance including fingerprinting
- Maintain acceptable driving record
- Carry $50,000 automobile liability insurance
- Ability to lift 20-30 pounds and assist unsteady clients
- Strong communication skills
- Skilled with Microsoft Office Suite
Job Qualifications
- Bachelor's degree in social work, psychology, social services, human services, behavioral sciences, or a related field
- Certified Medical Assistant (CMA) with an Associate's degree
- Significant experience related to mental health field without a degree
- At least two years of experience in the mental health field preferred
- Valid Washington state driver's license
- First Aid and CPR certification
- Licensure relevant to field of work
- Strong verbal and written communication skills
- Proficiency with Microsoft Office Suite
Job Duties
- Manage caseload of 50 clients meeting each client at least once per month
- Assess clients' presenting problems and formulate individualized health action plans
- Monitor and document clients' responses to interventions and revise plans regularly
- Link clients to health and social resources based on individual needs and preferences
- Accept, respond to, and process referrals from Lead Entities
- Conduct outreach activities and participate in face-to-face visits with clients at locations of their choosing
- Facilitate communication among primary care providers, medical specialists, and personal care providers
- Arrange transitional care from inpatient to other settings when necessary
- Report suspected incidents of abuse or neglect as a mandated reporter
- Maintain accurate and timely records in accordance with policies and regulations
- Support clients in achieving individualized health goals for recovery and improved functioning
