The role of the Care Concierge at Curana Health is vital within its Interdisciplinary Care Team. As an on-site representative at senior living facilities, the Care Concierge serves as a key liaison between patients, healthcare providers, and Curana’s virtual support team. This position involves direct patient interaction to monitor and support seniors enrolled in specialized programs like Advanced Primary Care Management (APCM) and Guiding an Improved Dementia Experience (GUIDE). The Care Concierge carries out monthly in-person rounds, handles inquiries from patients and their designated durable powers of attorney, assists with scheduling follow-ups, and responds to messages received through the Curana Patient Portal. On the provider support side, the role includes managing electronic health records (EHR), coordinating necessary documentation and provider orders, facilitating communication, and assisting with scheduling and prior authorizations. Communication and collaboration with the broader Curana Interdisciplinary Care Team are essential to ensure seamless coordination and continuity of care across the healthcare network.

This position typically requires on-site presence at assigned senior living communities for four business days per week and may involve travel between multiple locations. Candidates will need to demonstrate excellent organizational, time management, and computer skills, including proficiency in Microsoft Office applications. The role calls for flexibility, clinical knowledge, and a compassionate approach toward seniors and their caregivers. Curana Health offers a competitive hourly wage ranging from $20.00 to $24.00, depending on education, experience, and certification, coupled with a comprehensive benefits package including 401K, paid time off, and paid holidays subject to eligibility. Job Requirements

  • Registered or certified medical assistant
  • at least one year experience in medical office or related environment
  • ability to interpret medical records, lab results, and appointment notes
  • experience in collecting clinical data and communicating with care team
  • willingness to travel to senior living communities
  • proficient with Microsoft Office applications
  • strong organizational and time management skills

Job Qualifications

  • Registered or certified medical assistant
  • minimum one year experience in a medical office, community engagement, or related field
  • ability to interpret medical records, lab results, and appointment notes
  • experience collecting clinical information and reporting changes to care team
  • willingness to travel to local senior living communities
  • proficient computer skills including Microsoft Office
  • excellent organizational and time management skills
  • preferred experience with chronic care management or other care management programs

Job Duties

  • Conduct monthly in-person rounds for patients enrolled in Advanced Primary Care Management or the Guiding an Improved Dementia Experience programs
  • address patient and durable power of attorney inquiries via telephone
  • respond to patient or caregiver messages via Curana Patient Portal
  • assist patients with scheduling follow-up appointments with Curana Providers or specialists
