PACE of the Triad is a Program of All-Inclusive Care for the Elderly (PACE) organization dedicated to providing comprehensive, community-based healthcare services to older adults in Guilford and Rockingham counties in North Carolina.

Established in 2009, PACE of the Triad began serving participants in 2011, offering a holistic approach to healthcare that enables seniors to maintain their independence and quality of life within their own communities.

Facility

The organization operates from a renovated 18,000-square-foot health and human services facility located at 1471 E. Cone Blvd., Greensboro, NC 27405.

This center serves as the hub for PACE's services, which include:

• Complete Medical Care: Comprehensive healthcare services tailored to the unique needs of older adults, ensuring holistic well-being.
• Dental, Vision, and Hearing Services: Integrated care addressing all aspects of participants' health.
• Social Center: A vibrant community space offering social and recreational activities, fostering connections and engagement among participants.
• Home Care: Personalized in-home support to assist with daily activities, promoting independence and comfort.
• Exercise and Rehabilitation: Programs designed to maintain and improve physical health, including exercise classes and rehabilitation services.
• Transportation: Reliable transportation services to medical appointments and social center activities, ensuring accessibility.
• Medications, Equipment, and Supplies: Provision of necessary medications and medical supplies to support health management.

Partnership

In 2020, Lutheran Services Carolinas joined PACE of the Triad as an equity partner member organization, replacing Advanced Home Care.

This partnership aims to enhance services and strengthen the organization's community impact.
