Community Health Services, Inc. (CHS) is a private nonprofit Federally Qualified Health Center (FQHC) dedicated to providing comprehensive healthcare services to the Greater Hartford area in Connecticut.

Established in 1970, CHS has grown to become the state's second-oldest health center. It offers a wide range of medical, dental, behavioral health, and support services to meet the diverse needs of the community.

Medical Services

CHS offers a full spectrum of medical services, including:

• Pediatrics: Complete physicals for infants, school-aged children, and adolescents, as well as urgent medical care for sick visits.
• Adult Medicine: Comprehensive care for adults, addressing both acute and chronic health conditions.
• Women's Health: Services such as prenatal and postnatal care, ultrasounds, nutritional counseling, preventive health screenings, birth control counseling, STD screening and counseling, and treatment of abnormal pap tests.
• Behavioral Health: Mental health treatment, including individual and group therapy, medication management, and support for dual diagnosis treatment for co-occurring serious mental health illnesses and substance use disorders.

Dental Services

CHS provides comprehensive dental care, including:

• Preventive Care: Cleanings, X-rays, exams, sealants, and fluoride treatments.
• Restorative Services: Fillings, extractions, root canals, crowns, dentures, partials, and periodontal scaling.
• Emergency Care: Walk-ins are welcome for dental emergencies.
• Specialty Referrals: Referrals for oral surgery, endodontics, orthodontics, pedodontics, TMJ specialists, periodontics, and implantologists.

Additional Services

Beyond primary care, CHS offers:

• Laboratory Services: On-site lab testing for various health conditions.
• Pharmacy Services: Access to prescription medications and counseling.
• Podiatry: Foot and ankle care services.
• Optometry: Eye care services, including vision screenings and referrals.
• Food Pantry: A food pantry program available every other Friday, providing assistance to approximately 100 families. Emergency food assistance is also available Monday through Thursday.

CHS operates a primary location in Hartford:

• 500 Albany Avenue, Hartford, CT 06120 – This facility offers a wide range of services and operates Monday through Friday, with specific hours for each department.
