This travel opportunity is for a Skilled Nursing Facility (SNF) Physical Therapist in Portola Valley, California, designed for licensed professionals eager to make a significant and meaningful impact in patient care. This assignment spans 13 weeks, with a standard 40-hour work week consisting of eight-hour shifts, offering a pay range of approximately $2,456.87 to $2,532.86 per week. The role centers on assessing patients' physical abilities and crafting individualized treatment plans that improve mobility, strength, and overall functional independence. The Physical Therapist will collaborate closely with interdisciplinary healthcare teams to ensure holistic patient care, provide education to patients and their families on exercise programs and injury prevention, and monitor and document patient progress with adaptable treatment strategies. Additionally, the therapist will engage in discharge planning and coordinate follow-up care to facilitate continued rehabilitation and recovery. Job Requirements
- Doctorate in physical therapy
- Valid CA physical therapist license
- Current BLS/CPR certification
- Ability to work 40 hours per week
- Experience in skilled nursing facility settings preferred
- Strong communication and interpersonal skills
- Availability to start ASAP
