Job Requirements

  • Active RN license
  • Degree from accredited nursing program
  • Eighteen months of recent experience in an acute care case manager setting
  • BLS certification
  • Ability to communicate effectively with patients, families, and healthcare teams
  • Willingness to work 40 hours per week on 9-hour day shifts
  • Eligibility to work as a travel nurse in the United States

Job Qualifications

  • Degree from accredited nursing program
  • Active RN license
  • Eighteen months of recent experience in an acute care case manager setting
  • BLS certification
  • Strong communication and interpersonal skills
  • Ability to work independently and collaboratively in a fast-paced environment

Job Duties

  • Coordinate continuity of care for patients often as a liaison between the patient's family and healthcare organization
  • Strive to promote self-managed care and the use of healthcare resources in the most cost-effective way possible
  • Ensure that the proper treatment is administered at the appropriate time in order to maximize health and well-being
  • Communicate effectively with interdisciplinary healthcare teams to develop and implement patient care plans
  • Monitor patient progress and adjust care plans as necessary to meet individual needs
  • Facilitate discharge planning and ensure smooth transitions between care settings
  • Maintain accurate and timely documentation in compliance with healthcare regulations
