Job Requirements
- Active RN license
- Degree from accredited nursing program
- Eighteen months of recent experience in an acute care case manager setting
- BLS certification
- Ability to communicate effectively with patients, families, and healthcare teams
- Willingness to work 40 hours per week on 9-hour day shifts
- Eligibility to work as a travel nurse in the United States
Job Qualifications
- Degree from accredited nursing program
- Active RN license
- Eighteen months of recent experience in an acute care case manager setting
- BLS certification
- Strong communication and interpersonal skills
- Ability to work independently and collaboratively in a fast-paced environment
Job Duties
- Coordinate continuity of care for patients often as a liaison between the patient's family and healthcare organization
- Strive to promote self-managed care and the use of healthcare resources in the most cost-effective way possible
- Ensure that the proper treatment is administered at the appropriate time in order to maximize health and well-being
- Communicate effectively with interdisciplinary healthcare teams to develop and implement patient care plans
- Monitor patient progress and adjust care plans as necessary to meet individual needs
- Facilitate discharge planning and ensure smooth transitions between care settings
- Maintain accurate and timely documentation in compliance with healthcare regulations
