Job Requirements
- Valid unencumbered Michigan NP license
- minimum three years patient care experience
- experience in primary care or adult or geriatric care preferred
- proficiency in electronic medical records
- strong communication skills
- empathy and compassion
- traveling capability within Michigan
- ability to work independently in a home setting
Job Qualifications
- Valid unencumbered Michigan nurse practitioner license
- minimum of three years of patient care experience
- experience in primary care, adult, and geriatric medicine preferred
- proficiency with electronic medical records
- strong clinical judgment and assessment skills
- excellent written and oral communication skills
- ability to demonstrate empathy and compassion in patient care
Job Duties
- Conduct in-home health risk assessments including medical history evaluation, review of systems, medication reconciliation, vital signs, and physical exams
- perform evidence-based practice screening including point-of-care testing
- identify diagnoses for care management
- formulate a list of current and past medical conditions using clinical judgment
- collaborate with Porter Care Guides and medical management teams
- ensure compassionate communication with patients and coworkers
