The Nurse Practitioner role at Porter is a traveling position primarily focused on conducting comprehensive in-home health risk assessments for patients across Michigan, with the highest patient volumes concentrated in the Grand Rapids area. This position requires licensed nurse practitioners who are passionate about delivering personalized care and possess strong clinical judgment combined with a compassionate approach to patient management. Porter seeks candidates who can thoroughly evaluate patients by reviewing their medical history, conducting physical exams, and utilizing evidence-based screening tools to identify health risks and assist in the formation of effective care plans. This role offers competitive compensation based on patient assessments, with $110 per completed in-home assessment typically lasting 45-60 minutes, $65 per completed diagnostic visit of 20-30 minutes, and hourly rates ranging up to $110. Job Requirements
