The role in question is for a Travel Nurse Registered Nurse (RN) Case Manager specializing in Acute Care Case Management, based in Oakland, California. This travel nursing position is a 13-week assignment starting on August 31, 2026, with a work schedule of 40 hours per week over 8-hour evening shifts. As an essential component of the healthcare team, the RN Case Manager will coordinate continuity of care for patients, acting as a vital liaison between the patient's family and the healthcare organization. This role includes comprehensive case management responsibilities across various healthcare settings including HMOs, community health organizations, long-term care facilities, behavioral health programs, rehabilitation centers, and schools. The Case Manager RN strives to promote self-managed care, optimize the use of healthcare resources effectively, and ensure that patients receive the proper treatment at the right times to maximize their health and well-being.

Job Requirements

  • Degree from accredited nursing program
  • Active RN license
  • Eighteen months of recent experience in an Acute Care Case Manager setting
  • BLS certifications
  • Ability to work 40 hours per week on evening shifts
  • Willingness to travel to Oakland, California for 13 weeks
