The role of the Devoted Preventative Primary Care Provider is a critical part of Devoted Health's interdisciplinary clinical team. It is designed for Nurse Practitioners (NPs) or Physician Assistants (PAs) who are passionate about preventative care and improving patient outcomes through an innovative, technology-enabled environment. This role bridges preventative care, primary care, and complex care management, creating a unique and impactful opportunity that goes beyond standard primary care or annual wellness visit frameworks. Providers performing this role will conduct comprehensive clinical assessments, manage chronic conditions, focus on medication optimization, and deliver care to support longevity and quality of life for older adults.

Working primarily through telehealth video visits, with occasional in-person visits, providers will build long-lasting relationships with their patients, focusing on coordinated, member-centered care. This position emphasizes the use of advanced technology and AI-driven workflows to minimize administrative tasks and enhance the time spent on meaningful patient interactions. Providers collaborate closely with a diverse care team that includes specialists, pharmacists, nurses, caregivers, families, and care coordinators to ensure an individualized approach tailored to each patient’s unique needs.

The role offers standard schedules with options for flexible work arrangements, including the potential for a 4x10 schedule upon successful completion of the onboarding period. The salary range is competitive, ranging between $115,000 and $140,000 annually, with comprehensive benefits designed to support employees’ well-being and professional growth. Job Requirements

  • Active Nurse Practitioner (APRN/NP) or Physician Assistant (PA) license
  • 3+ years of outpatient clinical practice experience
  • Active and clear NP/PA license with willingness to obtain additional licenses if needed within 90 days
  • Active Basic Life Support (BLS) certification
  • Comfort with telehealth video visits and electronic health record systems
  • Ability to work collaboratively within a tech-enabled interdisciplinary team

Job Qualifications

  • Active Nurse Practitioner (APRN/NP) or Physician Assistant (PA) license
  • 3+ years of outpatient clinical practice, ideally in primary care, family medicine, internal medicine, or geriatrics
  • Active and clear NP/PA license (must be willing to obtain additional licenses if necessary within the first 90 days - support and reimbursement provided)
  • Active BLS certification at time of hire
  • Comfort with delivering care via video telehealth and using electronic documentation systems
  • Familiarity with managed care models, including STARS/HEDIS and identification of care gaps
  • Experience performing comprehensive or preventive care visits with Medicare populations
  • Experience supporting older adults and/or individuals with complex medical or social needs

Job Duties

  • Conduct primarily telehealth video visits, with limited and occasional in-home visits to members in your local area
  • Perform comprehensive assessments and provide evidence-based care focused on prevention, chronic condition support, medication optimization, and health maintenance
  • Identify and address care gaps, coordinate with external and internal care team members, and contribute to individualized, whole-person care plans
  • Utilize integrated technology and AI-enabled workflows to reduce administrative burden and maximize meaningful member interaction time
  • Communicate with empathy and clarity, fostering trust and confidence with members and caregivers
