The role of a Devoted Preventative Primary Care Provider, which includes Nurse Practitioners (APRNs/NPs) and Physician Assistants (PAs), is crucial to achieving the company's mission. This position is part of an interdisciplinary clinical team focused on preventative primary care, complex care management, and improving quality of life for older adults. It is distinct from traditional primary care roles and emphasizes building ongoing relationships with members starting from their initial stages of care. Providers conduct comprehensive clinical assessments, support chronic disease management, optimize medication use, and focus on preventative care to enhance health outcomes.

This role utilizes a tech-enabled and supportive environment designed to optimize clinical time and member connections, including telehealth video visits with occasional in-home visits. Providers work collaboratively with a team of specialists, pharmacists, nurses, care coordinators, caregivers, and family members to deliver coordinated, member-centered care.
