Job Description

The Coordinating Center (The Center) is a well-established nonprofit organization with over 40 years of experience dedicated to supporting children, youth, adults, and seniors who have complex medical needs and disabilities across the state. This is a full-time remote teleworking position located within The Coordinating Center’s Home and Community Coordination Services Division. The organization recognizes the importance of flexibility and autonomy in supporting its staff, providing hot spots and conference spaces at its Millersville, MD location for occasional in-person meetings and training sessions. The role of Coordination of Community Services (CCS) Team Manager is a leadership position responsible for supervising a team of CCS Coordinators who deliver targeted case management services to individuals with intellectual and developmental disabilities. The Team Manager ensures the delivery of person- and family-centered service coordination within the parameters of the CCS contract.

The CCS Team Manager carries a small caseload capped at 10 clients and serves a vital role in monitoring compliance with contract requirements, maintaining comprehensive documentation, and providing technical and analytical support related to contract planning, data extraction, and report writing. The Team Manager functions as a bridge between the CCS management team and frontline staff, driving quality outcomes and fostering excellence in service delivery. This individual leads by example, fostering a positive work environment, encouraging professional growth, and ensuring that staff members are trained, supported, and held accountable to the highest standards.

The position demands expertise in a variety of competency-based areas, including negotiation and conflict resolution, crisis management, community resource coordination, and compliance with developmental disabilities regulations. Proficiency in relevant computer applications such as Word, Excel, PowerPoint, and specific Long-Term Services and Supports (LTSS) documentation systems is required.

Job Requirements

  • Master’s degree with one year of experience in coordination of services or bachelor’s degree with three years of experience in coordination of services
  • Experience in social services intake and referral services
  • Experience in data collection, analysis, and reporting
  • Staff supervision or management experience
  • Knowledge of regulations for services to individuals with developmental disabilities
  • Proficiency in LTSS documentation system and Microsoft Office
  • Strong communication skills
  • Ability to manage multiple priorities and multitask

Job Qualifications

  • Master’s degree with one year of coordination of services experience or bachelor’s degree with three years’ coordination experience
  • Experience in social services intake and referral, data collection, analysis, reporting, staff supervision, management, or leadership
  • Competency in negotiation and conflict management, crisis management, community resources
  • Knowledge of the integrated settings appropriate to individual needs and service monitoring
  • Understanding of allied service delivery systems including Medicaid, mental health, substance abuse, social services, juvenile justice, vocational rehabilitation, and corrections
  • Familiarity with regulations governing services for individuals with developmental disabilities
  • Proficiency in LTSS documentation systems and Microsoft Office applications (Word, Excel, PowerPoint)
  • Strong communication skills including writing and speaking
  • Ability to manage complex work processes and multitask effectively
  • Demonstrated belief in opportunities for people with disabilities to participate fully in the community

Job Duties

  • Assume day-to-day coordination and oversight of the Coordination of Community Services (CCS) and provide direct supervision to team members
  • Monitor staff caseloads and ensures appropriate coverage
  • Ensure all contract requirements are met including person-directed supports and delivery of core services
  • Interview, orient, train, and mentor newly hired staff on CCS concepts, service delivery, and contract processes
  • Perform daily review of staff work to ensure billing compliance, provide feedback on documentation quality
  • Serve as a resource for problem solving client and contract issues, policy advisement, and staff support
  • Continually assess staff resources and training needs and communicate with Director for implementation
  • Ensure timely completion of performance evaluations and coaching sessions
  • Maintain a small client caseload to assist participants in selecting and monitoring services
  • Facilitate effective teamwork and maintain a positive work environment
  • Lead by example implementing The Center’s mission and values
  • Conduct regularly scheduled team meetings
  • Participate in mandatory trainings, workgroups, and committees
  • Manage multiple priorities with flexibility
  • Use data to inform decisions and improve services
  • Work collaboratively with internal and external partners to enhance comprehensive care
  • Ensure proper documentation and outcomes measurement in Care Management and LTSS systems
  • Complete other duties as assigned
