Disability Services & Advocacy (DS&A) is a dedicated organization committed to supporting individuals with intellectual and developmental disabilities in the state of New Jersey. As a statewide agency, DS&A operates through five regional teams dispersed across NJ, focusing on providing personalized support coordination services to people with disabilities to enhance their lives and help them navigate the complex social service systems in place. The role of a Support Coordinator at DS&A is a critical position designed to serve adults aged 18 and older with intellectual and developmental disabilities under the New Jersey Division of Developmental Disabilities (DDD) program. This position is predominantly remote, allowing coordinators to work from home with necessary regional travel to conduct field visits within their own communities. Support Coordinators are responsible for managing caseloads that vary depending on employment status, with full-time coordinators typically managing up to 50 consumers and part-time coordinators managing lower caseloads. This flexibility in workload supports both individuals seeking full-time engagement and part-time coordinators, including parents of adults with disabilities, looking to maintain expertise or gradually increase their involvement.

Support Coordinators at DS&A play a vital role in educating consumers and families about the individualized fee-for-service budget system, collaborating to create support and service plans that are realistic, budget-conscious, and aligned with consumer goals. Job Requirements

  • Bachelor's degree in any subject
  • minimum one year direct experience with adults 18 and older with intellectual or developmental disabilities
  • ability to pass fingerprinting, drug screening, and criminal background checks
  • must have reliable transportation for regional travel
  • submit cover letter, unofficial college transcripts, and current resume with documented experience
  • no substitute of academic degree in place of transcripts
  • experience working specifically with adults diagnosed with ID/DD
  • availability for mid-day hours and flexibility
  • ability to maintain minimum caseload
  • part-time or full-time availability
  • reside in the region served preferred

Job Duties

  • Explain to consumers and their families the fee-for-service individualized budget system
  • work with consumers and families to create support and service plans
  • monitor implementation of support plans through monthly calls, quarterly visits, and annual home visits
  • adjust plans according to changing needs and goals
  • assist consumers and families in navigating the disability support system
  • communicate compassionately and effectively with consumers and families
  • discuss financial and legal issues relevant to people with disabilities including guardianship, wills, and special needs trusts
  • input and update all support activity as required by DDD and DS&A
  • respond promptly to requests for information from consumers, families, providers, DDD officials, and DS&A management
  • utilize technology for communication and service plan preparation
  • represent DS&A professionally
