The Supportive and Palliative Care program at ChristianaCare is currently seeking a Social Worker II-MSW to join their dedicated team serving patients in Wilmington and Newark locations. This is a full-time, day-shift position Monday through Friday with no weekend or holiday work required. The role involves spending one to two days per week at the Wilmington site and the remaining time at the Newark site. The hourly pay range for this position is $30.34 to $48.55, depending on experience and qualifications.

The Social Worker II-MSW is an advanced-level role responsible for providing specialized case management and social services consultation to patients with serious illnesses, particularly those undergoing cancer treatment or living with chronic conditions. This position requires a compassionate professional able to build trusting relationships with patients and families, assessing social, emotional, and environmental needs. The social worker will develop comprehensive service plans that align with medical recommendations to enhance patient outcomes and overall well-being. In addition to direct casework, the role involves collaborating closely with interdisciplinary medical teams, coordinating between healthcare providers, patients, and community resources to deliver holistic care.

This role covers both inpatient and outpatient settings, demonstrating the program's commitment to continuous patient support throughout their healthcare journey. Key responsibilities include conducting emotional and social assessments, counseling individuals and families, evaluating financial eligibility for various supportive services, and maintaining detailed case documentation. The social worker will also provide expert consultations within the medical team, attend departmental meetings, and represent the supportive care program at external healthcare forums as needed.

Job Duties

  • Provides direct casework services which include emotional and social assessments
  • problem identification and develops a service plan to assist patient and family in dealing with medical conditions following medical recommendations and achieving maximum social and health adjustment
  • Assesses financial eligibility of families seeking financial, medical or related services
  • provides referral and follow up services to alternative sources for ineligible applicants
  • assists patients in obtaining same
  • Provides individual and family counseling to patients
  • obtains and coordinates public, private or community medical, social and therapy services to meet patient needs
  • Reviews patients’ case history and evaluates patients’ potential to benefit from further medical and social treatments and services
  • discusses case progress with multidisciplinary treatment team as required and prepares recommendations and referrals based on findings
  • Prepares social histories, patient progress reports, evaluations and summaries
  • prepares and maintains a variety of records and files pertaining to caseload and files
  • Provides medical social work consultations to patients, program staff, private medical professionals and/or hospital social service departments
  • Attends department and team meetings as requested
  • Represents Supportive and Palliative care at system and external meetings when requested by supervisor
  • Participates as a treatment team member to provide a social assessment and plan as part of the overall patient medical plan
  • Provides follow-up with patient and community resources to ensure coordination and consistency of medical/social treatment
  • Performs other related duties as required
