Job Requirements
- Master's degree in social work
- Unrestricted Arizona social work license or eligibility for provisional license
- Completion of BLS certification within 60 days of employment
- Two years of related social work experience in an acute care or case management setting
- Knowledge of social work theory and practice including DSM V
- Understanding of state and community support programs
- Knowledge of legal regulations regarding abuse reporting
- Strong self-starter with good communication skills
- Ability to maintain community resource database
- Basic computer skills including Word and Excel
- Valid Arizona driver's license and vehicle
Job Qualifications
- Master's degree in social work
- Unrestricted or provisional State of Arizona social work license
- Two years of social work experience in acute care, clinics, home health or case management
- Knowledge of social work theory and DSM V diagnostic tools
- Understanding of state and community-based program rules and regulations
- Familiarity with advanced directives, POLST, and legal reporting requirements
- Excellent communication and customer service skills
- Ability to build and maintain community resource databases
- Proficiency with Microsoft Word, Excel, and electronic medical records systems
- Valid Arizona driver's license and reliable vehicle
Job Duties
- Foster a workplace culture consistent with the organization's mission and values
- Provide direct and indirect patient care with exemplary customer service
- Assist with coordination of patient care under primary care provider guidance
- Act as liaison for health plan in person or by telephone
- Identify, assist, coordinate and initiate referrals for social service programs
- Formulate and manage social service plan of care throughout the care continuum
- Document assessments and services in electronic medical records including referral follow-ups
