Job Requirements
- Master’s level degree in Social Work or Behavioral Sciences
- One year of experience in healthcare or social work related field
- Valid current Oregon driver’s license and ability to meet SCHS driving requirements
- Ability to wear appropriate personal protective equipment
- Ability to perform physical requirements including sitting, standing, walking, and use of motor vehicle
- Compliance with all applicable laws, regulations, and policies
- Strong ethical conduct and professionalism
Job Qualifications
- Master’s level degree in Social Work or Behavioral Sciences
- One year of experience in healthcare or social work related field
- Knowledge of psychosocial assessment and therapeutic interventions
- Ability to collaborate effectively with interdisciplinary care teams
- Familiarity with community resources and social determinants of health
- Strong communication and documentation skills
- Valid current Oregon driver’s license
Job Duties
- Function as a member of the interdisciplinary care team to assure care is delivered efficiently
- Coordinate care plans collaboratively with interdisciplinary teams to address psychosocial needs related to injuries and critical incidents
- Advocate for patient needs to ensure access to appropriate services and resources for care and discharge planning
- Provide bio-psychosocial and trauma-informed interventions, support, and referrals to patients and families
- Conduct comprehensive assessments including economic, mental health, family dynamics, and social support systems
- Provide education and care planning for palliative care, hospice, and bereavement needs
- Identify victims of abuse and make appropriate referrals while complying with legal requirements
