The Social Services Specialist position within St. Charles Health System plays a critical role in supporting the psychosocial needs of patients and their families across various departments such as Emergency Room, Family Birthing, NICU, Pediatrics, Inpatient Medical Units, Inpatient Rehabilitation, Hospice, Home Health, and Behavioral Health. This position is relief, dayshift, with a pay range of $36.99 to $55.49 per hour, offering flexible scheduling with variable shifts and call requirements. Reporting to the Manager or Director of Care Coordination for Social Services, the specialist supports a multidisciplinary team by providing assessments, counseling, care coordination, and referrals to ensure patients receive optimal care throughout the continuum of their treatment journey.

The role requires the Social Services Specialist to perform comprehensive psychosocial assessments, provide therapeutic interventions, and coordinate appropriate services for patients facing medical, social, behavioral, and mental health challenges. You will work closely with physicians, nurses, chaplains, volunteers, and counselors to address the physical, emotional, mental, and spiritual needs of patients and families from admission through bereavement. The specialist advocates for patient rights, facilitates education, and assists with discharge planning by connecting patients to community resources and long-term care options. Responsibilities also include identifying victims of abuse, conducting substance abuse screenings, and supporting patients with palliative care and hospice needs. Job Requirements

  • Master’s level degree in Social Work or Behavioral Sciences
  • One year of experience in healthcare or social work related field
  • Valid current Oregon driver’s license and ability to meet SCHS driving requirements
  • Ability to wear appropriate personal protective equipment
  • Ability to perform physical requirements including sitting, standing, walking, and use of motor vehicle
  • Compliance with all applicable laws, regulations, and policies
  • Strong ethical conduct and professionalism

Job Qualifications

  • Master’s level degree in Social Work or Behavioral Sciences
  • One year of experience in healthcare or social work related field
  • Knowledge of psychosocial assessment and therapeutic interventions
  • Ability to collaborate effectively with interdisciplinary care teams
  • Familiarity with community resources and social determinants of health

  • Valid current Oregon driver’s license

Job Duties

  • Function as a member of the interdisciplinary care team to assure care is delivered efficiently
  • Coordinate care plans collaboratively with interdisciplinary teams to address psychosocial needs related to injuries and critical incidents
  • Advocate for patient needs to ensure access to appropriate services and resources for care and discharge planning
  • Provide bio-psychosocial and trauma-informed interventions, support, and referrals to patients and families
  • Conduct comprehensive assessments including economic, mental health, family dynamics, and social support systems
  • Provide education and care planning for palliative care, hospice, and bereavement needs
  • Identify victims of abuse and make appropriate referrals while complying with legal requirements
