The Social Services Consultant position at UAMS is a vital role within the ICE | PCPHSL Behavioral Health department, designed to offer crucial supportive services to patients and their care teams as they navigate treatment and assessment processes. This role is a regular, full-time daytime position that emphasizes collaboration with an interdisciplinary team comprising physicians, nurse practitioners, nurses, pharmacists, psychologists, and other healthcare professionals. The Social Services Consultant is tasked with conducting comprehensive psychosocial assessments to identify and address barriers impacting patient health, such as housing instability, food insecurity, transportation difficulties, financial challenges, caregiver stress, mental health concerns, and substance use. The consultant delivers brief counseling, crisis management, and support both to patients and families, contributing significantly to care continuity and health outcomes.

Responsibilities extend to coordinating referrals to community resources, social services, behavioral health providers, and specialty care, ensuring patients access necessary benefits including Medicaid, Medicare, disability services, and other community assistance programs. The consultant also participates actively in population health initiatives aimed at improving care for patients with complex medical and social needs, guiding patients through care transitions from hospital discharges to follow-up appointments. Proficiency in documenting patient interactions in electronic medical records in compliance with UAMS policies and HIPAA regulations is essential.
