Job Description

As part of its ongoing commitment, Vaya Health invites applications for the position of ROOTS Hub Initiative 5 Lead, a position based remotely but restricted to individuals residing in or near specific North Carolina counties, including Alexander, Anson, Cabarrus, Catawba, Cleveland, Gaston, Iredell, Lincoln, Mecklenburg, Rowan, Stanly, and Union, or within 40 miles of the North Carolina border. This residency requirement ensures proximity to the communities served and facilitates regular travel within the region as needed.
The ROOTS Hub Initiative 5 Lead serves as a pivotal figure within the Rural Health Transformation Program (RHTP), a statewide initiative targeting the enhancement of health access and outcomes. Reporting to the ROOTS Hub Initiatives Manager, this Lead is responsible for the implementation and operational oversight of Initiative 5 within region 3 of NCDHHS’s defined service area. The role is integral to advancing value-based payment readiness, healthcare sustainability, and the development of innovative financing models tailored to the needs of rural primary care providers and health systems. Through these efforts, the position supports the creation of rural community hubs designed to enhance prevention, chronic disease management, and workforce capacity development, ultimately fostering more sustainable healthcare environments.
The ROOTS Hub Initiative 5 Lead collaborates extensively with healthcare providers, payers, finance departments, implementation partners, local governments, and community stakeholders to drive operational transformation and improvement across rural healthcare delivery. The role necessitates a strategic approach to planning, stakeholder engagement, governance, community outreach, and performance monitoring to ensure the initiative meets its objectives and fulfills reporting obligations. This leadership role champions the establishment of connections between medical, behavioral health, and social services in rural areas to expand the availability and access to comprehensive healthcare.
The position demands a high level of emotional intelligence and discretion, along with exceptional communication skills tailored to diverse audiences. Candidates must possess strong project management and performance improvement expertise to contribute effectively to multi-faceted implementation plans and budgeting. The ability to navigate complex healthcare systems including Medicaid and Medicare, along with experience supporting healthcare sustainability in rural settings, is critical. Job Requirements
- Reside in North Carolina or within 40 miles of the NC border
- Bachelor’s degree in public health, healthcare administration, business administration, finance, or related field
- Minimum of two years of experience project managing large and complex engagements
- Minimum of two years of experience coordinating across different types of stakeholders
- Minimum of two years of experience in healthcare systems, Medicaid and/or Medicare payer systems understanding
- Strong project management and operational planning skills
- Ability to coordinate across healthcare systems and stakeholder groups
- Ability to manage multiple priorities and deadlines
- Strong written and verbal communication skills
- Experience working with Medicaid and Medicare beneficiaries preferred
- Certifications in project management methodologies preferred
- Certification in quality and process improvement preferred
Job Qualifications
- Bachelor's degree in public health, healthcare administration, business administration, finance, or related field
- Minimum of two years of experience project managing large and complex engagements
- Minimum of two years of experience coordinating across different types of stakeholders
- Minimum of two years of experience in healthcare systems, Medicaid and/or Medicare payer systems understanding
- Strong project management and operational planning skills
- Knowledge of value-based care, healthcare financing, rural healthcare systems, and population health principles
- Ability to coordinate across healthcare systems and stakeholder groups
- Strong analytical, facilitation, and communication skills
- Knowledge of Medicaid, Medicare, and healthcare reimbursement structures preferred
- Proficiency in Microsoft Office applications
Job Duties
- Lead day-to-day implementation activities related to Initiative 5 priorities, action plans, and deliverables
- Support rural provider readiness for value-based care and alternative payment models
- Coordinate sustainability planning and operational transformation activities
- Monitor initiative milestones, deliverables, and operational performance
- Coordinate implementation activities among healthcare, behavioral health, and social service partners
- Identify implementation barriers and recommend operational improvements
- Establish ROOTS Hub that connects medical, behavioral health, and social services to improve availability and access to healthcare in rural communities
