Job Description
Vaya Health is a leading organization dedicated to improving health outcomes for individuals living in rural and underserved communities across North Carolina. As part of the North Carolina Department of Health and Human Services (NCDHHS), Vaya Health administers the Rural Health Transformation Program (RHTP), a statewide initiative strongly supported by the US Centers for Medicare & Medicaid Services (CMS). This ambitious program aims to ensure sustainable improvements in access to healthcare and patient outcomes by focusing on evidence-based and community-informed interventions tailored to rural populations. By working closely with healthcare providers, public health agencies, federally qualified health centers, community organizations, and regional stakeholders, Vaya Health plays a critical role in strengthening the preventive care systems essential to improving the quality of life for rural residents.
The ROOTS Hub Initiative 2 Lead position is a remote role requiring residency within North Carolina or within 40 miles of the NC border and preferably near certain specified counties such as Alexander, Anson, Cabarrus, Catawba, Cleveland, Gaston, Iredell, Lincoln, Mecklenburg, Rowan, Stanly, or Union counties. This strategic leadership role reports directly to the ROOTS Hub Initiative Manager and is responsible for supporting the implementation and operational oversight of Initiative 2 within NCDHHS Region 3. Initiative 2 emphasizes enhancing prevention and screening services, chronic disease management, maternal health, and nutritional health by expanding successful evidence-based programs throughout rural communities in the region.
In this role, the ROOTS Hub Initiative 2 Lead will manage a variety of critical functions, including fostering collaborative partnerships with healthcare providers, behavioral health partners, and social service agencies to create more accessible, effective care models. The Lead will contribute to developing rural community hubs that connect medical, behavioral, and social services, thereby addressing healthcare disparities while focusing particularly on rural populations. Responsibilities also include coordinating engagement with lead anchors such as Appalachian State University, AppWell, Office of Rural Promise, Partners Behavioral Health Management, Cabarrus Health Alliance, and Catawba County Public Health to ensure that program activities align with region-specific priorities and NCRHTP performance objectives.
The position demands a strong combination of project management, performance monitoring, community engagement, and reporting skills. The Lead must skillfully navigate complex healthcare systems, including Medicaid and Medicare payer structures, and possess a deep understanding of rural health challenges, population health principles, and chronic disease prevention. Frequent collaboration and communication with diverse stakeholder groups require high emotional intelligence, discretion, and the ability to tailor messages effectively to a wide audience.
Apart from project and operational duties, the ROOTS Hub Initiative 2 Lead also plays a critical role in quality improvement by monitoring milestones, assessing barriers to implementation, recommending solutions, and supporting corrective action planning. In addition, the Lead supports governance activities, contributes to strategic planning, and may serve as a subject matter expert on behalf of Initiative 2 when needed. This leadership position is pivotal to advancing the RHTP’s goal of creating sustainable, sustainable health systems that better serve the rural population of North Carolina.
The salary for this exempt position will be commensurate with the qualifications and experience of the candidate. Job Requirements
- Bachelor’s degree in public health, healthcare administration, nursing, social work, nutrition, or related field
- Minimum of two years of experience project managing large and complex engagements
- Minimum of two years of experience coordinating across different types of stakeholders
- Minimum of two years of experience in an organization or field demonstrating understanding of healthcare systems, Medicaid and/or Medicare payer systems, clinical and social needs affecting community access to health services
- Residency in North Carolina or within 40 miles of the NC border
- Ability to travel across specified North Carolina counties
- Strong written and verbal communication skills
- Proficiency in Microsoft Office applications
- Ability to manage multiple priorities and deadlines
- High degree of emotional intelligence and discretion
Job Qualifications
- Bachelor’s degree in public health, healthcare administration, nursing, social work, nutrition, or related field
- Master’s degree preferred
- Minimum of two years of experience project managing large and complex engagements
- Minimum of two years of experience coordinating across different types of stakeholders
- Minimum of two years of experience in an organization or field demonstrating understanding of healthcare systems, Medicaid and/or Medicare payer systems, clinical and social needs affecting community access to health services
- Experience serving or working on behalf of Medicaid and/or Medicare beneficiaries preferred
- Population health experience preferred
- Experience with physical health, behavioral health, I/DD and TBI populations preferred
- Certifications in project management methodologies preferred
- Certification in quality and process improvement preferred
Job Duties
- Lead day-to-day implementation activities related to Initiative 2 priorities
- Coordinate prevention, chronic disease management, maternal health, and nutrition-related initiatives
- Support expansion and scaling of evidence-based health improvement programs
- Monitor initiative milestones, deliverables, and operational performance
- Coordinate implementation activities among healthcare, behavioral health, and social service partners
- Identify implementation barriers and recommend operational improvements
- Establish ROOTS Hub that connects medical, behavioral health, and social services to improve availability and access to healthcare in rural communities
- Coordinate within the organization and with lead anchor partners to ensure all Initiative 2/RHTP responsibilities and requirements are met
- Support the ROOTS Hub Initiatives Manager to track Vaya’s RHTP plan’s compliance and performance against program-related deadlines and milestones
- Support development of program-related reports as outlined by Requirements from NC DHHS
- Support region 3 anchors and RHTP subrecipients, implementation partners, and community-based organizations in understanding contracting, invoicing, documentation, reimbursement, reporting, and compliance expectations
- Build and maintain collaborative relationships with healthcare providers, public health agencies, community organizations, and implementation partners
- Support incorporation of community feedback and regional health priorities into implementation activities
- Participate in community engagement activities and ensure community voice is incorporated into implementation planning
- Partner closely with project team, finance department, lead anchors and community stakeholders to ensure the milestones and deliverables for RHTP
- Foster relationships with rural communities and disproportionately burdened populations
- Support project plan and provide regular updates to ROOTS Hub Initiatives Manager regarding RHTP successes and challenges
- Attend and/or lead RHTP Initiative 2 related meetings
- Participate in community health assessments to support the development of needed services
- Monitor initiative performance metrics and outcomes
- Assist with preparation of reports, dashboards, presentations, and implementation updates
- Identify opportunities for improvement and support corrective action planning as needed
- Support compliance with NCDHHS requirements and reporting expectations
- Collaborate with other Initiative Leads and ROOTS Hub staff to ensure alignment across initiatives
- Participate in Hub governance and planning activities
- Serve as Initiative 2 subject matter expert as requested
