Job Description

Vaya Health is a leading healthcare organization committed to improving the lives of North Carolina residents, especially those living in rural and underserved communities. As a statewide health entity, Vaya Health is dedicated to advancing integrated care coordination, enhancing access to essential medical, behavioral, and social services, and fostering sustainable health outcomes across the region. Through collaborative partnerships with community-based organizations, health providers, and public health departments, Vaya Health is at the forefront of driving innovative solutions to complex healthcare challenges in rural areas. The organization emphasizes equity, cultural competence, and community engagement in its operations to ensure that all populations, including those disproportionately burdened by social determinants of health, receive the care and resources they need. Employing forward-thinking healthcare models and leveraging data-driven strategies, Vaya Health actively supports regional health transformation initiatives that align with the priorities of local populations and state healthcare goals.

The current opportunity is for the position of ROOTS Hub Initiative 1 Co-Lead within the Rural Health Transformation Program (RHTP), which is administered by the North Carolina Department of Health and Human Services (NCDHHS) and funded by the US Centers for Medicare and Medicaid Services (CMS). This role is remote with the residency requirement to live in or near specified North Carolina counties or within 40 miles of the NC border. The Co-Lead reports to the ROOTS Hub Implementation Manager and plays a critical leadership role in advancing Initiative 1 objectives, focusing on building and strengthening ROOTS Hub infrastructure that seamlessly connects medical, behavioral health, and social service systems across a defined Region 3 consisting of Alexander, Anson, Cabarrus, Catawba, Cleveland, Gaston, Iredell, Lincoln, Mecklenburg, Rowan, Stanly, and Union counties.

The Co-Lead is a strategic partner responsible for overseeing day-to-day implementation efforts, coordinating among diverse stakeholders, and ensuring adherence to program milestones and performance metrics. This includes operational planning, stakeholder and community engagement, governance support, and monitoring implementation activities to promote integrated systems of care for rural populations. The position requires excellent project management skills, strong communication abilities tailored to a wide array of partners, and an understanding of healthcare systems including Medicaid and Medicare frameworks. It also demands a high level of emotional intelligence, discretion, and independent judgment, given the sensitive and complex nature of rural health transformation work. The role involves collaboration with regional lead anchors such as Appalachian State University’s Office of Rural Promise, Partners Behavioral Health Management, and local public health agencies to tailor the program’s interventions according to regional rural health needs and performance objectives.

The ROOTS Hub Initiative 1 Co-Lead is instrumental in facilitating governance bodies, fostering community and stakeholder relationships, supporting program compliance and reporting, and continuously enhancing the initiative’s impact. The work includes preparing meeting materials, tracking program compliance, supporting budgeting and contracting processes for subrecipients, and participating actively in community health assessments to align service development with local needs. This leadership role provides an exceptional opportunity to contribute to a far-reaching program dedicated to transforming rural healthcare delivery in North Carolina. The position is exempt, salaried, and offers a chance to make a meaningful difference by improving healthcare access, coordination, and outcomes for rural and vulnerable populations. Interested candidates are encouraged to apply through Vaya Health’s career center. Vaya Health is an equal opportunity employer committed to inclusive hiring practices and community representation.
