The RN Health and Social Services Care Coordinator position with UnitedHealthcare is a vital role within their integrated care model, designed to support long-term and community-based care needs. Located in Oahu, HI, this position provides the flexibility to work remotely while requiring up to 75% local travel within the assigned region, including areas such as downtown Honolulu, the West Side, East Side, and North Shore. The role demands strong collaboration with social service programs, community resources, and clinical teams to address the financial, psychosocial, and medical needs of Medicaid members. This position offers a competitive hourly wage ranging from $28.27 to $50.48 based on experience and qualifications, along with a $5,000 sign-on bonus for external candidates.
