The RN Health and Social Services Care Coordinator position with UnitedHealthcare is a vital role within their integrated care model, designed to support long-term and community-based care needs. This role emphasizes the importance of personalized health care, ensuring that patients receive the most appropriate and least restrictive level of care tailored specifically to their individual needs. As a Care Coordinator, you will be a key advocate and liaison between patients, families, and healthcare providers to facilitate seamless transitions and effective management throughout the continuum of care.

Located in Oahu, HI, this position provides the flexibility to work remotely while requiring up to 75% local travel within the assigned region, including areas such as downtown Honolulu, the West Side, East Side, and North Shore. The role demands strong collaboration with social service programs, community resources, and clinical teams to address the financial, psychosocial, and medical needs of Medicaid members. As part of a dynamic and elite team, you will leverage your nursing expertise and community health knowledge to implement care strategies, manage care plans, and advocate for patients’ best interests effectively.

This position offers a competitive hourly wage ranging from $28.27 to $50.48 based on experience and qualifications, along with a $5,000 sign-on bonus for external candidates. Job Requirements

  • Current, unrestricted RN license in the state of Hawaii
  • 2+ years of clinical experience
  • Experience working directly or collaborating services for long-term care, home health, hospice, public health or assisted living
  • Intermediate level of proficiency with MS Word, Excel and Outlook
  • Ability to travel in assigned region to visit Medicaid members in their homes and or other settings, including community centers, hospitals or providers offices
  • Driver's license and access to reliable transportation
  • Current access or ability to obtain internet access via a landline

Job Qualifications

  • Current, unrestricted RN license in the state of Hawaii
  • 2+ years of clinical experience
  • Experience working directly or collaborating services for long-term care, home health, hospice, public health or assisted living
  • Intermediate level of proficiency with MS Word, Excel and Outlook
  • Ability to travel in assigned region to visit Medicaid members in their homes and or other settings, including community centers, hospitals or providers offices
  • Driver's license and access to reliable transportation
  • Current access or ability to obtain internet access via a landline
  • Bachelor's degree or higher
  • CCM (Certified Case Manager)
  • Experience with or exposure to discharge planning
  • Experience with utilization review, concurrent review and/or risk management
  • Experience with electronic charting
  • Experience with arranging community resources
  • Field-based work experience
  • Case management experience
  • Experience working within the community health setting
  • Background/experience with at least one of the following: behavioral/mental health, psychiatric nursing, housing/homeless outreach, community resource linkages, pregnancy, pediatrics or disease management
  • Background in managing populations with complex medical or behavioral needs
  • Background in mental health or experience working with serious mental illness
  • Bilingual in Cantonese or Mandarin
  • Psych background including community nursing such as PHN or IP psych facility

Job Duties

  • Assess, plan and implement care strategies that are individualized by patient and directed toward the most appropriate, least restrictive level of care
  • Identify and initiate referrals for social service programs including financial, psychosocial, community and state supportive services
  • Manage the care plan throughout the continuum of care as a single point of contact
  • Communicate with all stakeholders the required health-related information to ensure quality coordinated care and services are provided expeditiously to all members
  • Advocate for patients and families as needed to ensure the patient's needs and choices are fully represented and supported by the health care team
