This specific opportunity is within Corewell Health's Concierge Medicine program at the Care Center located on Lake Drive in Grand Rapids. The Concierge Medicine model emphasizes personalized care, holistic health assessments, and tailored coaching to meet individual patient needs. Unlike traditional practices, Corewell’s concierge practice limits the number of patients per provider, allowing for more thorough, attentive, and personalized care. The Nurse role in this setting is critical, supporting the clinical care team with patient communication, care management, and coordination of services such as scheduling and referrals. This position is part-time with 20 hours per week, primarily during day shifts from 8:00 a.m. to 5:00 p.m., Monday through Friday.

Job Requirements

  • Associate's degree in nursing required
  • Licensed Registered Nurse (RN) in Michigan upon hire
  • Certification in Basic Life Support (BLS) required within 90 days
  • Minimum one year of nursing experience preferred
  • Ability to work part-time 20 hours per week, Monday to Friday with variable schedule
  • Commitment to comply with Corewell Health's vaccine and drug-free workplace policies
  • Strong interpersonal and communication skills
  • Ability to collaborate effectively in a multidisciplinary team environment

Job Qualifications

  • Associate's degree in nursing required
  • Bachelor's degree in nursing preferred
  • Licensed Registered Nurse (RN) in the State of Michigan upon hire
  • Certification in Basic Life Support (BLS) from American Heart Association or American Red Cross within 90 days of hire
  • Minimum one year of nursing experience preferred
  • Care management background strongly preferred

Job Duties

  • Provide care management services to support patient health and wellness
  • Assist clinical care team with patient communication including appointment scheduling and follow-ups
  • Deliver direct patient care including assessments, diagnostics, and health education
  • Coordinate referrals and collaborate with healthcare providers to ensure seamless patient care
  • Use nursing process to assess, diagnose, plan, implement, and evaluate patient care
  • Uphold standards of nursing excellence and comply with American Nurses Association code of ethics
  • Participate in team meetings and contribute to care planning and quality improvements
