The role of the Hospice Clinical Liaison at Gentiva is pivotal in ensuring the intake and referral processes run smoothly to meet the needs of patients, families, referral sources, and the organization as a whole. This experienced and compassionate position requires a professional to build strong relationships with healthcare providers and community partners, coordinate admissions efficiently, and enhance community awareness of hospice care services. As a Hospice Clinical Liaison, you will be responsible for assessing patient eligibility, educating families, managing documentation, and acting as the crucial communication link between referral sources and the hospice care team. The role involves daily visits to hospitals and healthcare facilities to maintain strong, visible connections with discharge planners, case managers, and physicians. Individuals in this role must demonstrate excellent clinical, communication, and organizational skills, alongside a deep commitment to patient advocacy and quality care.

This position requires a licensed nurse with significant hospice experience, ideally someone with a Bachelor of Science in Nursing (BSN) and a minimum of three to five years in hospice settings. The candidate should be comfortable managing a variety of responsibilities, including staff supervision, service coordination, and program management, while working sensitively with diverse patients and families. Job Requirements

Job Qualifications

  • Licensed nurse with three to five years of hospice experience
  • BSN with five or more years of hospice experience preferred
  • Minimum of three years of hospice experience
  • Prior experience as a discharge planner, case manager, or educator preferred
  • Current RN license to practice nursing in the state of employment
  • CPR certification
  • Valid driver's license and automobile insurance
  • Strong knowledge of hospice care and patient needs
  • Familiarity with community resources and referral networks
  • Excellent patient assessment and documentation skills
  • Ability to supervise staff, coordinate services, and manage programs effectively
  • Strong communication and interpersonal skills with sensitivity to diverse backgrounds

Job Duties

  • Receive and respond to referrals from hospitals, home health, physicians, case managers, skilled nursing facilities, and other healthcare professionals
  • Assess patient eligibility for hospice services by reviewing medical records, conducting clinical evaluations, and communicating with physicians and families
  • Educate patients and families on hospice care, providing clarity, reassurance, and compassion
  • Complete necessary documentation and coordinate the intake and admission process, ensuring all legal, clinical, and payer requirements are met
  • Serve as a liaison between referral sources and the hospice care team ensuring seamless handoffs and clear communication
  • Make daily visits to hospitals and facilities maintaining visibility with discharge planners, case managers, and physicians
  • Build and maintain strong relationships with referral partners and identify new outreach opportunities to increase awareness of hospice services
