The role of the Hospice Clinical Liaison at Gentiva is pivotal in ensuring the intake and referral processes run smoothly to meet the needs of patients, families, referral sources, and the organization as a whole. This experienced and compassionate position requires a professional to build strong relationships with healthcare providers and community partners, coordinate admissions efficiently, and enhance community awareness of hospice care services. As a Hospice Clinical Liaison, you will be responsible for assessing patient eligibility, educating families, managing documentation, and acting as the crucial communication link between referral sources and the hospice care team. The role involves daily visits to hospitals and healthcare facilities to maintain strong, visible connections with discharge planners, case managers, and physicians. Individuals in this role must demonstrate excellent clinical, communication, and organizational skills, alongside a deep commitment to patient advocacy and quality care.

This position requires a licensed nurse with significant hospice experience, ideally someone with a Bachelor of Science in Nursing (BSN) and a minimum of three to five years in hospice settings. The candidate should be comfortable managing a variety of responsibilities, including staff supervision, service coordination, and program management, while working sensitively with diverse patients and families.
