We are currently seeking an experienced and compassionate Hospice Clinical Liaison to join our dedicated team. This role is critical in ensuring an efficient and effective intake and referral process that meets the needs of patients, families, referral sources, and our organization. As a Hospice Clinical Liaison, you will serve as the cornerstone of communication between referral sources, such as hospitals, physicians, and case managers, and our hospice care team. Your responsibilities will include assessing patient eligibility through clinical evaluations and medical record reviews, educating patients and families about hospice care, and managing admission coordination to ensure all legal and payer requirements are met. This role requires daily visits to hospitals and healthcare facilities to maintain strong visibility and accessibility, fostering robust professional relationships that promote community awareness of hospice services. You will play a vital role in guiding families with clarity, compassion, and support during some of their most challenging moments.

This position requires a compassionate individual with strong interpersonal skills, excellent clinical knowledge, and the ability to work collaboratively within an interdisciplinary team. Ideal candidates will hold a current Registered Nurse license, possess a minimum of three years of hospice experience, and have a demonstrated commitment to patient advocacy and quality care.
