Gentiva is seeking an experienced and compassionate Hospice Clinical Liaison to join their team. This full-time role plays a critical part in the intake and referral process for hospice services, ensuring efficiency and quality in meeting the needs of patients, families, and referral sources. The Hospice Clinical Liaison acts as a key connector between healthcare providers and the hospice care team, facilitating clear communication, timely admissions, and community awareness. Responsibilities include receiving referrals from hospitals, home health, physicians, case managers, and skilled nursing facilities, assessing patient eligibility for hospice, educating patients and families on hospice care, and coordinating all documentation and admission requirements. The liaison also makes daily visits to hospitals and facilities, builds relationships with referral partners, and identifies new outreach opportunities to enhance hospice service utilization. This role requires a licensed nurse with significant hospice experience, strong clinical and communication skills, and a compassionate approach toward terminally ill patients and their families.

Compensation for this role ranges from $75,610 to $94,515 per year, depending on various factors including location, experience, and certifications.
