We are currently seeking an experienced and compassionate Hospice Clinical Liaison to join our team. This role is vital in ensuring that intake and referral processes are conducted efficiently to meet the needs of patients, families, referral sources, and the organization. The Hospice Clinical Liaison serves as an essential bridge between hospitals, home health agencies, physicians, case managers, skilled nursing facilities, and our hospice care team. In this position, you will assess patient eligibility for hospice services, educate patients and families about hospice care, and coordinate admissions by completing all necessary documentation in compliance with clinical, legal, and payer requirements. This role requires a professional who excels in building and maintaining strong relationships with referral partners and community stakeholders to increase awareness of hospice services. Daily visibility within healthcare facilities such as hospitals and skilled nursing centers is an important aspect of the role to establish rapport with discharge planners, case managers, and physicians.

The ideal candidate will be a licensed nurse with substantial experience in hospice care and possess strong clinical assessment skills, excellent communication abilities, and a compassionate approach to patient advocacy. The Hospice Clinical Liaison acts as an educator and advocate, supporting patients and families through a sensitive and often emotional journey while ensuring seamless coordination of care.
