This particular role is for an experienced Nurse Practitioner or Physician Assistant to join CCFM’s fully remote clinical team. The position offers the flexibility and convenience of working from a home office while being part of a dynamic, nationally recognized team led by functional medicine experts. The role comes with a part-time schedule initially, with the potential for full-time expansion, and competitive hourly compensation ranging from $70 to $85 based on experience. As a member of this clinical team, you will conduct comprehensive virtual visits for patients with complex, multi-layered health concerns. Your responsibilities will include thorough new patient evaluations, follow-up care, development of individualized treatment plans addressing root causes, and ongoing monitoring and adjustments based on clinical progress. Collaboration with fellow clinicians for case discussions, contributing to clinical protocol refinement, and utilizing advanced technologies such as AI-assisted documentation tools are integral to the role. Beyond clinical duties, you will also contribute to patient education efforts through handouts, workshops, and mentorship of clinicians in affiliated fellowship programs.

Job Requirements
- Licensed nurse practitioner or physician assistant in California
- Clinical experience or strong interest in functional medicine
- Experience or interest in managing mold-related illness
- Experience or interest in tick-borne disease and Lyme disease
- Knowledge or interest in hormone optimization and autoimmune conditions
- Familiarity with gut health, adrenal dysfunction, and immune system support
- Ability to interpret conventional and functional diagnostic testing
- Access to a quiet professional home office
- Reliable high-speed internet
- Appropriate computer equipment for telemedicine
- Self-motivated and able to work independently
Job Qualifications
- Active California Nurse Practitioner or Physician Assistant license
- MD and DO applicants will also be considered
- Experience in functional medicine such as IFM or ADAPT training preferred
- Strong clinical reasoning and problem-solving skills
- Excellent communication and patient education abilities
- High emotional intelligence and compassionate approach to patient care
- Self-motivated with ability to work independently in a remote environment
- Strong organizational and time management skills
- Comfortable using telemedicine platforms, Google Workspace, Zoom, and AI documentation tools
Job Duties
- Conduct thorough new patient evaluations and follow-up appointments via telemedicine
- Develop individualized treatment plans that identify and address underlying causes of illness
- Manage patients with complex chronic conditions involving multiple overlapping diagnoses
- Monitor patient progress and adjust care plans based on clinical response
- Educate patients to keep them informed, supported, and empowered throughout their care
- Collaborate with fellow clinicians on difficult cases and participate in regular clinical discussions
- Help refine clinical protocols and best practices as new evidence emerges
