Job Requirements
- Associate degree in nursing or higher
- active, unrestricted RN license in state of practice
- minimum 2 years of home health or hospice RN experience
- previous supervisory or leadership experience preferred
- Homecare Homebase experience preferred
- experience with Microsoft Office
- ability to learn enterprise systems such as Workday
- reliable transportation
- valid driver's license
- current auto insurance
- ability and willingness to travel within assigned territory
- physical ability to lift, push, pull, and support up to 50 pounds including transferring patients and moving equipment
Job Qualifications

Job Duties
- Oversee daily patient care operations and clinical staff assignments
- coordinate referrals, patient care needs, and care plan implementation
- ensure timely patient admissions and compliance with regulatory standards
- review and approve physician orders and plans of care
- conduct OASIS reviews and maintain quality and compliance standards
- coach, mentor, and support field clinicians
- participate in interviewing and hiring clinical staff
- monitor key branch performance metrics and partner with leadership on operational goals
- coordinate communication among clinicians, physicians, patients, and families
- support field visits as needed
