The role of Registered Nurse (RN) Home Health Manager at Elara Caring is a critical leadership position based in Floyds Knobs, Indiana. Job Requirements

  • Associate degree in nursing or higher
  • active, unrestricted RN license in state of practice
  • minimum 2 years of home health or hospice RN experience
  • previous supervisory or leadership experience preferred
  • Homecare Homebase experience preferred
  • experience with Microsoft Office
  • ability to learn enterprise systems such as Workday
  • reliable transportation
  • valid driver's license
  • current auto insurance
  • ability and willingness to travel within assigned territory
  • physical ability to lift, push, pull, and support up to 50 pounds including transferring patients and moving equipment

Job Qualifications

Job Duties

  • Oversee daily patient care operations and clinical staff assignments
  • coordinate referrals, patient care needs, and care plan implementation
  • ensure timely patient admissions and compliance with regulatory standards
  • review and approve physician orders and plans of care
  • conduct OASIS reviews and maintain quality and compliance standards
  • coach, mentor, and support field clinicians
  • participate in interviewing and hiring clinical staff
  • monitor key branch performance metrics and partner with leadership on operational goals
  • coordinate communication among clinicians, physicians, patients, and families
  • support field visits as needed
