The Registered Nurse (RN) Home Health Manager position at Elara Caring is based in Chelmsford, Massachusetts, offering a significant opportunity for an experienced clinical leader to join an innovative team. As a Clinical Team Manager, the selected candidate will play an instrumental role in overseeing patient care and clinical personnel in a home health setting, ensuring that patients receive timely, diagnosis-specific, and individualized care plans. This managerial role requires dedication to quality and compliance, as well as the ability to coordinate effectively with physicians, vendors, and internal teams. With a base salary range of $95,000 to $110,000 annually, this staff position also offers potential additional compensation through performance-based bonuses and other incentives. The role involves a combination of administrative oversight and hands-on clinical leadership, including regular patient assessments, staff management, and direct support in the field as needed.

Job Requirements

  • Associate degree in a nursing related field required
  • 3-5 years of skilled home health experience including field visits and case management
  • Experience with Oasis documentation and assessments
  • 1 year of supervisory experience preferred
  • Current unrestricted RN license valid for the state of work
  • Ability to quickly become proficient in enterprise applications such as Homecare Homebase, Workday, and have knowledge in Microsoft Office
  • Able and willing to travel within branch or office coverage area
  • Must have a dependable vehicle, valid driver’s license, and current auto insurance in accordance with state laws
  • Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment

Job Qualifications

  • Associate degree in a nursing-related field
  • 3-5 years of skilled home health experience including field visits and case management
  • Experience with OASIS documentation and assessments
  • 1 year of supervisory experience preferred
  • Current unrestricted RN license valid for Massachusetts
  • Ability to quickly become proficient in enterprise applications such as Homecare Homebase and Workday
  • Knowledge of Microsoft Office
  • Strong communication and leadership skills
  • Dependable vehicle with valid driver’s license and auto insurance in accordance with state laws

Job Duties

  • Provide oversight of all patient care services and personnel including making patient and personnel assignments and coordinating patient care
  • Coordinate referrals and assure patient needs are continually assessed
  • Assure development, implementation, and updates to individualized patient plans of care
  • Plan, direct, and participate in delivery of home health services for diagnosis-specific patient care
  • Coordinate services with providers, vendors, or facilities
  • Ensure timely start of patient care within regulatory guidelines
  • Process and coordinate physician orders, staff assignments, and communications among care team members, patients, families, and physicians
  • Review and approve plans of care and evaluate proposed changes for clinical appropriateness
  • Integrate orders from all relevant physicians and ensure approval
  • Conduct regular OASIS reviews for all patients
  • Meet regularly with direct care team members to provide guidance and coaching
  • Initiate hiring requests, interview candidates, and recommend hiring of direct care staff
  • Communicate frequently with Branch Director or Administrator on key performance metrics
  • Remain available for field work as needed
  • Perform other duties or projects as assigned
  • Handle a physically demanding, high-stress environment
