Your role is critical in supporting patients in their home environments, enhancing their health outcomes while maintaining the comfort of familiar surroundings.

- ability to work independently and manage time efficiently
Job Duties
- Provide independent, exceptional care to patients in their homes including comprehensive assessments and creating individualized care plans
- document care in a timely and thorough manner using HomeCare HomeBase EMR
- perform Start of Care, Discharge, Resumption of Care/Recertification, and Revisit visits according to OASIS standards
- communicate effectively with patients, families, and healthcare teams to coordinate care
- adhere to all compliance and regulatory requirements relevant to home health nursing
- utilize clinical judgment to identify changes in patient condition and intervene appropriately
- maintain professional development through ongoing education and mentorship
