Every day, more than 10,000 VNS Health team members deliver unparalleled expertise along with 24/7 health solutions and resources to over 43,000 patients who rely on their care. Powered by exceptional data analytics capabilities—unmatched in the home and community-healthcare industry—VNS Health offers a comprehensive array of health care services, solutions, and health plans designed to streamline the health care experience and to address the wide-ranging, complex needs of the diverse communities they serve in New York and beyond.
The role in question focuses on supporting clinical operations and facilitating effective interactions among various stakeholders including business support, Quality Management Systems (QMS), and Certified Home Health Aide (CHHA) clinical operations. This position plays a vital role in supporting the scheduling, documentation, and call-handling functions, ensuring smooth communication between patients, clinicians, and clinical leadership. The professional works under general direction, providing cross-regional support to all CHHA programs and other B-Pods, assisting teams by creating clinician schedules, triaging cases, and escalating critical issues to clinical management as needed.
Key responsibilities include troubleshooting operational and clinical issues, responding to emergent clinical calls, coordinating care communications, and maintaining compliance with agency guidelines and regulatory standards. The professional also collaborates closely with clinical managers to resolve complex cases and serves as an essential support system for field staff. Additional duties involve training and guiding other staff, engaging in quality improvement initiatives through documentation reviews, and facilitating communication among internal and external parties involved in audits and record requests. This position may occasionally involve providing direct patient care during emergent or special circumstances.
Vocationally, this role demands a Registered Nurse license in the state of New York, with a strong foundation in home health care delivery, knowledge of Medicare, Medicaid, and third-party payers, and proficiency in team leadership and group facilitation. The salary offered ranges from $119,258 to $145,458 per year, with a per diem per visit option ranging from $69.34 to $110. This role is integral to ensuring that clinician performance, patient care quality, and operational workflow meet the high standards set by VNS Health, ultimately supporting its mission of compassionate, home-based health care.
