The Registered Nurse (Center Nurse) role at Oak Street Health is an essential clinical care team position that operates within a Value-Based Care model. The Center Nurse plays an indispensable role in driving quality care for patients by performing clinical assessments, managing chronic conditions, and collaborating with interdisciplinary teams to reduce avoidable hospitalizations. Reporting directly to the Practice Manager, the Center Nurse is responsible for both walk-in and telephonic triage, ensuring patients receive appropriate care levels and avoid unnecessary hospital visits. Responsibilities include executing protocol-driven chronic condition management, performing skilled nursing procedures such as wound care and medication administration (including vaccines), and providing comprehensive education to patients and caregivers about chronic conditions and preventative care. Medication review during nurse-led visits is integral to ensuring patient safety and medication adherence.

In addition to direct patient care, the Center Nurse conducts onsite testing including point-of-care labs, EKGs, and specimen collection, using patient data to identify and close gaps in preventative screenings while ensuring compliance with state scope of practice regulations. The role involves reviewing and acting on abnormal laboratory results promptly to maintain patient safety and continuity of care. The Center Nurse also partners with providers and the interdisciplinary team to manage complex care plans and coordinate weekly transitions for patients moving from hospital to clinic settings.

This position requires utilizing standardized protocols for medication management, prescription renewals, and prior authorizations. Effective clinical communication management, timely completion of tasks, and closure of patient visit encounters are critical duties. The Center Nurse delegates tasks to medical assistants and phlebotomists as needed to support patient flow and operational efficiency, in addition to performing other related duties as assigned.

Clinically, the nurse supports the coordination and implementation of nursing practices and standard operating procedures while identifying barriers to optimal care such as physical, cognitive, language, literacy, socioeconomic, cultural, and behavioral factors. Creating a welcoming environment and delivering exceptional patient experiences aligned with respectful care and responsiveness are also priorities. The Center Nurse serves as a critical clinical voice in daily care team meetings, supports nursing staff and providers, ensures compliance with patient safety initiatives, and provides emergency care when needed.
