This role is a full-time, on-site position based at the West Allis, WI branch location, requiring a dedicated leader who is passionate about home health nursing and clinical management. As a Clinical Manager, you will play a pivotal role in overseeing the clinical operations of the branch, ensuring that patient care delivery meets the highest standards of quality, safety, and compliance. Reporting directly to the Branch Director, you will be responsible for guiding and supporting a team of skilled clinicians, including registered nurses and therapists, helping them excel in their roles while maintaining strong documentation practices and regulatory adherence.
Your leadership will extend to reviewing referrals for admission appropriateness, assigning clinicians to cases, and ensuring that Plans of Care are comprehensive and personalized to meet the needs of each patient. You will also provide ongoing education and clinical support, helping your team make informed clinical decisions and meet performance goals. Monitoring audits, billing compliance, and case management quality are essential parts of your role, along with active participation in hiring, training, and performance management to cultivate a motivated and capable clinical staff.
As a key communicator, you will facilitate collaboration between physicians, team members, and caregivers to coordinate care effectively and support discharge planning and positive patient outcomes. The Clinical Manager will additionally engage in quality improvement initiatives, data tracking, budgeting, marketing activities, and building strong community relationships to enhance the branch's presence and service quality.
This position also requires a readiness to provide direct patient care in exceptional circumstances and to step in as Branch Director when needed. The role demands flexibility, strong clinical acumen, and excellent leadership skills to manage a busy branch with a census of approximately 156 patients and maintain its 4.5-star rating.
The work schedule is standard business hours from Monday to Friday, 8 AM to 5 PM Central Time, with an on-call requirement to handle urgent matters as they arise. The starting annual salary ranges between $77,200 and $106,200, reflecting experience, skills, and geographic location. In addition to a competitive base salary, the role is eligible for an annual incentive bonus plan based on company and individual performance.

Job Requirements
- Valid driver's license, auto insurance and reliable transportation
- Two years as a Registered Nurse with at least one year of management experience in a home care, hospice or equivalent environment
- Home health experience
- Management and people leadership experience
- OASIS experience
- Graduate of an accredited School of Nursing
- Current state license as a Registered Nurse
- Proof of current CPR

Job Duties
- Oversee clinical operations including patient care delivery, staff management, documentation quality, and regulatory compliance
- Review referrals, determine admission appropriateness, assign clinicians, and ensure Plans of Care meet patient and agency standards
- Guide, support, and educate clinicians including goal-setting, care planning, and clinical decision-making
- Ensure clinical documentation, audits, and billing comply with Medicare, payer, and company standards
- Participate in hiring, training, performance evaluation, coaching, and corrective action for clinical staff
- Conduct ongoing staff education based on documentation review, utilization review findings, and performance improvement data
- Coordinate communication among physicians, team members, and caregivers to support care coordination, discharge planning, and outcome achievement
