The role available at DMC Sinai-Grace Hospital is for a Clinical Care Coordinator within the nursing department. This is a full-time position with a night shift schedule, offering an exciting opportunity for a registered nurse to take on leadership responsibilities in addition to providing patient care. As a Clinical Care Coordinator, you will serve as a key link between nursing staff and hospital administration, ensuring excellent care delivery and the smooth operation of unit activities during your shift.

The Clinical Care Coordinator assumes responsibility for coordinating the care delivery team during their shift. This includes scheduling and assigning nursing staff, prioritizing and delegating patient care responsibilities, and evaluating the quality of care provided. The role involves collaborating with interdisciplinary teams to resolve unit and system issues, participating in process improvement initiatives, and contributing to staff performance evaluations. Additionally, the Clinical Care Coordinator supports staff education and professional development, facilitating in-service programs and assisting in interviewing, mentoring, and retaining nursing personnel.

This position requires a nurse who is both a clinical resource and a leader, capable of managing unit throughput to improve patient length of stay (LOS) while promoting a collaborative and efficient work environment. The Clinical Care Coordinator is expected to maintain clinical competence by taking on a patient care assignment during their shift, ensuring they remain closely connected to the direct care activities of the unit. Job Requirements

  • Associates degree or diploma required
  • Licensed to practice as a registered nurse by the state of Michigan
  • One to two years of progressively more responsible experience with leadership abilities
  • Demonstrated ability to prioritize work, delegate to others and facilitate processes
  • Minimum Level 6 of the Promoting Excellence Performance Criteria preferred for internal candidates
  • BLS certification required
  • ACLS certification preferred

Job Qualifications

  • Associates degree or diploma required
  • BSN preferred
  • Licensed to practice as a registered nurse in Michigan
  • One to two years of progressively responsible experience with leadership abilities
  • Demonstrated ability to prioritize work and delegate
  • Minimum Level 6 of Promoting Excellence Performance Criteria preferred for internal candidates
  • BLS required, ACLS preferred
  • Clinical knowledge and critical thinking skills
  • Effective interpersonal and leadership skills
  • Familiarity with basic database computer software
  • Effective verbal and written communication skills

Job Duties

  • Assumes shift responsibilities for the care delivery team
