The Referral Triage Coordinator role is an essential position at SHSC, focused on managing and evaluating potential organ donor referrals. This role supports the organization’s strategic goals by initiating, guiding, and monitoring the medical management of organ donors with oversight from the Administrator On-Call (AOC) and the Medical Director. The Coordinator plays a vital role in ensuring donor suitability by performing clinical evaluations, screening, and collaborating with various stakeholders to protect donor and recipient safety. This position also involves providing education to hospital staff and other professionals to promote awareness and understanding around donation processes.
This full-time role requires a high level of clinical expertise, professionalism, and excellent interpersonal skills to foster positive relationships with hospital personnel, donor families, and healthcare providers. The Referral Triage Coordinator is responsible for responding promptly to potential organ donor cases, conducting thorough medical record reviews, and ensuring compliance with legal, medical, and ethical standards. A deeper understanding of death declaration protocols, organ donor consent procedures, and donor management protocols is crucial to execute the duties effectively.
The ideal candidate will have experience in critical care nursing, paramedic medicine, or related medical fields and possess strong computer skills, including proficiency with Microsoft Office and electronic medical record systems. Preferred qualifications include certifications such as Certified Procurement Transplant Coordinator (CPTC) and Certified Transplant Preservationist (CTP). The role involves on-call responsibilities, requiring the ability to handle cases for extended periods, sometimes up to 24 consecutive hours.

Job Requirements

Job Qualifications
- Associate degree or higher in nursing, respiratory therapy, paramedic medicine, related biological science (including transplant and donation science), or medical school degree (including foreign degree programs) preferred
- Bachelor’s degree or higher in nursing, respiratory therapy, or related biological science preferred
- critical care nursing, paramedic medicine, medical technician or related medical experience preferred
- proficiency in Microsoft Office products (Word, Excel, PowerPoint, etc.)
- proficiency in Microsoft Outlook
- preferred experience with electronic medical records such as Cerner, Epic, Meditech
- certified Procurement Transplant Coordinator (CPTC) preferred
- certified Transplant Preservationist (CTP) preferred
Job Duties
- Maintain the highest standards of professional conduct and interpersonal relationship development
- respond on-site to all potential organ donors in a timely manner unless prior approval given by Administrator on Call (AOC)
- perform clinical screening and evaluation to determine donor suitability for donation after circulatory death (DCD) or brain death donation (BDD) in collaboration with AOC
- complete referral evaluations to determine organ donor suitability based on NDN, CMS, OPTN/UNOS, AOPO, and CDC guidelines
- complete hospital medical record review including history of present illness, past medical history, malignancies, and surgical interventions/treatments
- demonstrate understanding of legal and hospital requirements for death declaration and organ donation
- guide referring agency to ensure informed consent is requested by designated requestor in accordance with organizational and hospital policy
- provide hospital-based education in collaboration with hospital development staff
- work collaboratively with Family Service Counselor (FSC) to obtain informed consent and medical/social history from donor next-of-kin per SHSC guidelines
- identify organ-specific and behavioral risk factors in accordance with SHSC and regulatory guidelines
- collaborate with Coroner and Medical Examiner’s office to ensure jurisdictional medical and legal requirements for death and organ recovery are satisfied
- initiate donor management protocols according to SHSC guidelines
- understand and comply with procedural aspects and requirements of blood, culture, and typing specimen collection and labeling
- complete, maintain, and ensure quality of all data forms required for donation process
- maintain confidentiality on all donor-related activities and internal issues
- utilize resources with discretion and employ professional laterality
- perform other duties as requested by Manager of Clinical Services, Director of Clinical Services, President and CEO, and/or SHSC Medical Director
- perform other duties as assigned
