Job Description
Tandigm Health is a transformational leader in population health management committed to improving health outcomes through innovative care coordination and quality improvement initiatives. Based in the greater Philadelphia area, Tandigm Health partners with healthcare providers, payers, and practices to deliver integrated population health programs designed to enhance patient care quality and operational efficiency. Known for its collaborative culture, Tandigm Health fosters an environment where employees feel valued, empowered, and motivated to make a significant impact in healthcare delivery. The company emphasizes superior customer service, care quality, and a commitment to health equity across the patient populations it serves. The organization offers competitive compensation and benefits, supporting a healthy work-life balance and professional growth.
We are currently seeking a Temporary Quality Clinical Coordinator (QCC) to join our Clinical team on a full-time basis. This position is a remote opportunity but requires that candidates reside within the Greater Philadelphia area. The QCC will play a crucial role in supporting quality initiatives related to patient medication adherence and closing Stars quality gaps. The role involves direct outreach to patients to encourage medication adherence, handling administrative tasks such as patient scheduling, coordinating with primary care practices and pharmacies, and documenting detailed outcomes using electronic medical records and quality platforms. The QCC also provides support to practices in developing effective processes to achieve and maintain 4+ Stars performance.
In addition to patient outreach, the Temporary Quality Clinical Coordinator is responsible for various quality-related functions that contribute to the effectiveness and success of the quality team. This includes assisting with quality projects, collaborating with healthcare providers to address complex patient needs, and leveraging community resources to overcome barriers to care. The position requires excellent communication and interpersonal skills, a strong attention to detail, and proficiency with electronic medical records and Microsoft Office. The Temporary QCC must also adhere to HIPAA standards to protect patients' protected health information (PHI) at all times.
This role suits candidates with a background in healthcare settings such as physician practices, hospitals, pharmacies, or health plans, and who have demonstrated customer service excellence. The ideal candidate is organized, proactive, resourceful, and able to work independently while also functioning well within a team environment. This opportunity offers a chance to contribute meaningfully to quality improvement initiatives and enhance patient outcomes in a supportive and mission-driven organization.
