The position at hand is a Regional Grant Coordinator role, focused on supporting Indiana’s 5-year statewide Rural Health Transformation Program (RHTP). Specifically, this role centers on the Make Rural Indiana Healthy Again (MRIHA) Regional Grants initiative. These grants represent a significant multi-year investment dedicated to enhancing rural healthcare access, prevention efforts, and strengthening health system resilience throughout Indiana. The role is full-time and hybrid, providing flexibility with partial remote work while supporting hands-on coordination within assigned rural counties. The coordinator plays a pivotal role in managing regional contracts, ensuring compliant execution of grants, and helping regional stakeholders meet clearly defined milestones and deliverables set forth by the Indiana Department of Health (IDOH). This position requires frequent travel within the designated region, which includes Knox, Daviess, Martin, Gibson, Pike, Dubois, Posey, Vanderburgh, Warrick, or Spencer counties, and occasional travel to Indianapolis for in-person meetings.
The Regional Grant Coordinator will act as the key liaison between the IDOH and regional coalitions such as hospitals, clinics, EMS providers, community organizations, and other partners. They will promote inclusive representation of rural populations, ensuring initiatives align with local priorities while fostering strong communication channels with the statewide grant team. The role involves actively supporting regional stakeholders with interpreting state program guidance, managing budgets and reporting requirements, and resolving challenges to maintain momentum and meet objectives. This position also includes oversight of CMS reporting requirements, regional data collection, and aligning performance metrics to federal standards. The coordinator will collaborate closely with technical assistance providers and participate in regional and consortium meetings, facilitating cross-region learning and sharing best practices.
Candidates must have a bachelor’s degree in public health, health administration, public policy, business administration, or a related field, with five years of experience in grant management, contract oversight, or program administration. They should also have healthcare or public health experience in rural community settings, a strong ability to manage multiple projects, excellent communication and relationship-building skills, and must reside within the assigned region to ensure consistent travel and presence. Experience with CMS-funded programs and familiarity with Indiana’s rural healthcare landscape is essential. The role requires a high level of independence due to the remote field-based nature, combined with collaborative engagement across multiple stakeholders.

Job Duties
- Participate in regular check-ins, statewide meetings, and required trainings
- Serve as the primary regional contract manager for organizations receiving RHTP funding
- Monitor compliance with state and federal requirements, including allowable expenditures, budget adherence, and documentation standards
- Support IDOH in ensuring regional coalitions meet milestones, deliverables, and timelines outlined in the state-defined workplan
- Identify risks, performance issues, or compliance concerns and escalate to IDOH as appropriate
- Maintain audit-ready documentation and ensure consistent use of state-approved templates, tools, and reporting systems
- Collaborate closely with statewide Technical Assistance Providers to ensure aligned support for regional coalitions
- Participate in regional meetings convened by Technical Assistance Providers, including consortium meetings, workgroups, and application development sessions
- Facilitate cross-region learning by sharing insights, challenges, and best practices
- Coordinate regional data collection, performance tracking, and outcome measurement in alignment with CMS Rural Health Transformation Program requirements
- Ensure timely submission of all required regional reports to IDOH for state and federal reporting cycles
- Support regional partners in understanding and meeting CMS reporting expectations, including documentation, metrics, and narrative requirements
- Serve as the day-to-day liaison between IDOH and regional coalitions, including hospitals, clinics, EMS providers, community organizations, and other partners
- Promote broad representation of rural populations and ensure regional activities reflect local needs and priorities
- Maintain strong communication channels with IDOH’s Regional Grant Initiative Team and participate in statewide meetings as requested
- Assist regional partners in interpreting state guidance, funding requirements, and program expectations
- Track regional progress, identify barriers, and support problem-solving to maintain momentum
- Contribute to statewide consistency by using shared tools, templates, and evaluation frameworks
