Job Description
MENTOR Adult Day Health, part of the Sevita family, is a reputable provider of specialized day programs designed to support elders with complex medical or rehabilitative needs. Established with a commitment to enhancing the quality of life for seniors, MENTOR enables participants to receive high-quality healthcare while remaining active, engaged, and connected to their communities. The organization recognizes the importance of allowing seniors to live independently in their own homes, promoting autonomy and dignity. With a seasoned team comprising qualified licensed nurses and experienced caregivers, MENTOR ensures personalized and compassionate care through individually tailored assistance to every participant.
The role of Program Case Coordinator at the Q1 Adult Day Health Center is a crucial position within this supportive environment. This position is designed to facilitate seamless coordination of services for Adult Day Health participants by maintaining clear and precise documentation, assisting in admissions processes, and promoting continuity of care among internal teams and external community providers. A unique aspect of this opportunity is the requirement for bilingual Spanish-English candidates, underscoring the organization's dedication to cultural competency and effective communication.
As a Program Services Case Coordinator, you will serve as a vital link between participants, their families, healthcare professionals, and community resources. Your responsibilities will include overseeing participant programs aimed at fulfilling their clinical, social, and community integration needs. This involves reviewing evaluations, progress notes, and discharge documentation to ensure individual care goals are met and properly documented. Additionally, the coordinator acts as the primary point of contact for families, offering clear communication, guidance, and support in navigating healthcare services.
The position offers competitive compensation, hourly pay between $23 and $24, along with a comprehensive benefits package that includes medical, dental, and vision insurance. The role adheres to a consistent weekday schedule from Monday through Friday, with no work required on weekends, fostering a healthy work-life balance. Sevita’s full-time employees benefit from paid time off that begins from the first day of employment, paid holidays, a 401(k) retirement plan with company match, and employee referral bonuses. Furthermore, employees have access to an expansive employee discount program featuring over 30,000 discounts.
Working at MENTOR Adult Day Health means joining a collaborative and supportive team where each member’s contribution is valued. The organization prides itself on fostering a mission-driven work culture that emphasizes compassion, reliability, responsibility, and a commitment to improving participant outcomes. This role is ideal for individuals passionate about making a tangible difference in the lives of others, especially those with disabilities or complex medical needs, through person-centered care and community inclusion. If you possess strong organizational skills, attention to detail, and excellent communication abilities, including bilingual fluency in English and Spanish, this position offers a meaningful career path in healthcare coordination within a well-established and respected organization.
