The Primary Care Physician (PCP) role at ChenMed is central to the organization's mission to provide superior care and achieve optimal health outcomes for its patients. This role entails full accountability for the clinical care, cost-effectiveness, and overall health outcomes of a patient panel typically consisting of up to 450 patients. The PCP is expected to develop trusting relationships with patients, functioning as a health coach who guides patients toward setting and achieving meaningful health goals through behavior change and consistent follow-up. This proactive approach aims to improve the quality of life and health status of each patient through comprehensive assessment, diagnosis, treatment, and ongoing management of acute and chronic illnesses.

The PCP acts not only as a clinician but also as a leader within their care team, supervising care promoters, facilitators, and coordinators to ensure coordinated and high-quality care delivery. For patients unable to visit the office, the PCP collaborates with transitional care teams and other healthcare professionals to maintain continuity of care. The PCP is also accountable for engaging with hospitalists during patient hospitalizations and supports collaborative care agreements with Advanced Practice Practitioners, maintaining responsibility for overall patient outcomes.

ChenMed maintains an outcomes-focused, value-based healthcare model with specific metrics used to measure PCP success, such as patient admissions, appointment slot utilization, patient experience scores (CGCAHPS), clinical gap closures, and medical cost management. PCPs are expected to work closely with center and market leadership to meet or exceed these goals while fostering a positive organizational culture.

Candidates must demonstrate strong competencies including exceptional availability to patients, service orientation, evidence-based medicine, leadership skills, quality improvement commitment, and the ability to influence patients and teams positively. Proficiency in Microsoft Office and other relevant software is required, along with the flexibility to travel locally and nationally.

ChenMed offers competitive compensation, with a salary range from $221,141 to $315,915 annually, depending on experience and location. Additional bonuses or commissions may also apply.
