With a panel size of up to 450 patients, Primary Care Physicians (PCPs) at ChenMed are empowered to influence their patients’ health outcomes significantly by building trusting relationships and coaching patients to achieve short- and long-term health goals. PCPs at ChenMed act as health coaches rather than just consultants for sickness, working closely with care teams to simplify and prioritize care, ensuring that behavioral change is actionable and sustainable.

PCPs are held accountable for various key performance metrics including patient admissions per thousand, appointment slot utilization, patient experience scores (CGCAHPS), clinical gap closures, and medical cost management. They are instrumental in fostering a positive culture of love, accountability, and passion within their medical centers, promoting teamwork among care providers. The role requires physicians to work both independently and collaboratively with other healthcare professionals, including medical directors, managers, and advanced practice practitioners, ensuring high standards of patient care, compliance with regulatory guidelines, and active participation in quality and risk management programs.

Key responsibilities of the PCP include comprehensive management of patients with acute and chronic illnesses, coordinating care with hospitalists and transitional care teams, supervising care promoters, facilitators, and coordinators, and actively participating in recruitment and center management initiatives. This role demands strong leadership, clinical expertise, and commitment to the continuous improvement of medical practice and patient outcomes.

Job Requirements

  • Must be able to obtain or have current active State Medical License(s)
  • Flexibility to work evenings, weekends and/or holidays as needed
  • Use and exercise independent judgment

Job Qualifications

  • MD or DO in Internal Medicine, Family Medicine, Geriatrics or similar specialty
  • Board eligibility required, board certification preferred
  • Ability to maintain board certification through MOC, CME or retaking exams
  • Current DEA number for schedule II-V controlled substances
  • Basic Life Support certification within 90 days of employment
  • Fluent in English verbal and written
  • Proficient in Microsoft Office Suite and other relevant software
  • Ability and willingness to travel locally and occasionally regionally or nationally
  • Demonstrated leadership skills
  • Commitment to evidence-based medicine and patient-centered care

Job Duties

  • Provide care independently for patients with acute and chronic illnesses encountered in older adults
  • Take full accountability for patient care and outcomes while seeking specialist consultation when needed
  • Engage with hospitalists for hospitalized patients regardless of employer
  • Assess, diagnose, treat, manage, educate, promote health and coordinate care for patients with complex health needs
  • Lead and collaborate with care team including medical assistants, care facilitators and coordinators
  • Engage with transitional care teams and case managers for patients unable to visit the office
  • Participate in center management and assist in recruiting patients and additional providers
  • Supervise and collaborate with Advanced Practice Practitioners while remaining accountable for their actions
  • Perform other duties as assigned by management
