The Primary Care Physician (PCP) role at ChenMed is pivotal in carrying out the organization's mission. The PCP manages a panel of up to 450 patients, primarily older adults, providing comprehensive care that combines clinical excellence with compassionate coaching. This role goes beyond traditional disease treatment by focusing on setting achievable health goals with patients and simplifying medical action plans. PCPs at ChenMed are responsible for numerous critical performance metrics, such as patient admissions, appointment utilization, patient satisfaction scores (CGCAHPS), clinical gap closures, and managing medical costs efficiently. PCPs at ChenMed must exhibit strong leadership and collaborative skills by working closely with managers, medical directors, and other healthcare professionals. Job Requirements

  • Medical degree from accredited institution
  • valid medical license to practice
  • board certification in primary care or relevant specialty
  • minimum clinical experience in primary care focusing on geriatric or adult populations
  • ability to manage a patient panel of up to 450 patients
  • strong commitment to patient accessibility and service orientation
  • proficiency in evidence-based medical practices
  • ability to lead and coordinate multidisciplinary care teams
  • excellent interpersonal and communication skills
  • fluency in English
  • proficiency with Microsoft Office

Job Duties

  • Provide care independently for acute and chronic illnesses in older adults
  • take full accountability for patient care and outcomes, seeking specialist consultation as needed
  • engage with hospitalists for hospitalized patients
  • manage assessment, diagnosis, treatment, education, and care coordination for patients
  • lead and collaborate with care team members including care promoters, facilitators, and coordinators
  • work with transitional care teams for patients in hospital, skilled nursing, long-term care, or homebound
  • participate actively in center management, covering for absent providers and assisting with recruitment
  • perform additional duties as assigned by management
