The Primary Care Physician (PCP) role at ChenMed is pivotal in carrying out the organization's mission. The PCP manages a panel of up to 450 patients, primarily older adults, providing comprehensive care that combines clinical excellence with compassionate coaching. This role goes beyond traditional disease treatment by focusing on setting achievable health goals with patients and simplifying medical action plans. PCPs at ChenMed are responsible for numerous critical performance metrics, such as patient admissions, appointment utilization, patient satisfaction scores (CGCAHPS), clinical gap closures, and managing medical costs efficiently. PCPs at ChenMed must exhibit strong leadership and collaborative skills by working closely with managers, medical directors, and other healthcare professionals. This position requires a high level of independence, balanced by teamwork within interdisciplinary care teams consisting of care promoters, facilitators, and coordinators. PCPs also extend their care beyond the clinic to hospitals, skilled nursing facilities, long-term care settings, and patient homes, collaborating with transitional care teams and other specialists to ensure continuity and quality of care.

This position offers a competitive salary range from $221,141 to $315,915 annually, depending on factors such as experience, education, and location.
